Abstract
Introduction
Clinicalpracticeshouldbeinformedbyhighqualityevidence,ofwhichrandomisedcontrolledtrials(RCTs)areconsideredthegoldstandard.Surgicaltrialsareinherentlydifficultwithpotentialproblemsaroundclinicalequipoiseandparticipantacceptability.Thisisoftenmosttruewithtrialdesignscomparingoperativeandnon-operativetreatments.Itishopedthatresearchactivitycanbemaximisedbycollaboratingin(a)theidentificationofresearchquestionsand(b)involvementinclinicaltrials.Developmentofthenationalresearchnetworkscanbeutilisedtoprovidesupportforresearchendeavourswithintheorthopaedictraumacommunity.
Aims
Toidentifyandprioritisetheresearchquestionsfelttobeofmostimportancebytheorthopaedictraumacommunity.Researchstudieswillbeconsideredforquestionsgiventhehighestpriority.
Methods
ADelphiapproachwasusedtodetermineconsensusbetweenthefacultymembersoftheAOUK.Atworoundprocesswasusedtoelicittheresearchquestionsandthentoranktheminorderofpriority.
Results
217membersoftheAOUKLongBoneFacultywereaskedtosubmitresearchquestions,predominantlyconsultantorthopaedicsurgeons.A22%responserategenerated147questions.Thesewerecollatedandthemostfrequent24sentbackoutforrankingbymeanscores.A55%responsetothissecondroundidentified10topquestions.Literaturesearchesforthese10lookedatcurrentknowledgeofthesubject,completedandongoingresearchprojects.Wealsolookedattheadvantagesanddisadvantagesofundertakingastudyandthemostappropriatemethodology.
Conclusion
Theresponseratesdemonstratedaclearinterestindevelopingacollaborativeresearchstrategy.ThiscanbeenhancedbyutilisingthesupportoftheNationalInstituteofHealthResearchClinicalResearchNetworks(NIHRCRN).

OpenMRS(www.openmrs.org)isaconfigurableopensourceelectronicmedicalrecordapplicationdevelopedandmaintainedbyalargenetworkofopensourcedeveloperscoordinatedbytheRegenstriefInstituteandPartnersinHealthandmainlyusedforHIVpatientandtreatmentinformationmanagementinAfrica.OurobjectiveistodevelopanopenImplementersNetworkforOpenMRStoprovideregionalsupportforthegrowingnumberofOpenMRSimplementationsinAfricaandtoincludeAfricandevelopersandimplementersinthefuturegrowthofOpenMRS.
Methods
WehavedevelopedtheOpenMRSImplementersNetworkusingadedicatedWikisiteande-mailserver.WehavealsoorganizedannualmeetingsinSouthAfricaandregionaltrainingcoursesatAfricanlocationswhereOpenMRSisbeingimplemented.AnOpenMRSInternshipprogramhasbeeninitiatedandwehavestartedcollaboratingwithsimilarnetworksandprojectsworkinginAfrica.Toevaluateitspotential,OpenMRSwasimplementedinitiallyatonesiteinSouthAfricabyasingleimplementerusingadownloadableOpenMRSapplicationandonlytheOpenMRSImplementersNetworkforsupport.
Results
TheOpenMRSImplementersNetworkWikiandlistserverhavegrownintoeffectivemeansofprovidingimplementationsupportandforumsforexchangeofimplementationexperiences.TheannualOpenMRSImplementersmeetinghasbeenheldinSouthAfricaforthepastthreeyearsandisattractingsuccessivelylargernumbersofparticipantswithalmost200implementersanddevelopersattendingthe2008meetinginDurban,SouthAfrica.SixAfricandevelopersarepresentlyregisteredonthefirstintakeoftheOpenMRSInternshipprogram.SuccessfulcollaborationshavebeenstartedwithseveralAfricandevelopergroupsandprojectsinitiatedtodevelopinteroperabilitybetweenOpenMRSandvariousapplications.TheSouthAfricanOpenMRSImplementergroupsuccessfullyconfigured,installedandmaintainedanintegratedHIV/TBOpenMRSapplicationwithoutsignificantprogrammingsupport.Sincethen,thismodelhasbeenreplicatedinseveralotherAfricansites.
Conclusions
TheOpenMRSImplementersNetworkhascontributedsubstantiallytothegrowthandsustainabilityofOpenMRSinAfricaandhasbecomeausefulwayofincludingAfricansinthedevelopmentandimplementationofOpenMRSindevelopingcountries.TheNetworkprovidesvaluablesupportandenablesabasicOpenMRSapplicationtobeimplementedintheabsenceofonsiteprogrammers.

Objective

Findings
AMRNcanbedescribedasasmall,dispersed,looselycoupledprofessionalnetworkwhichhasmadeconsiderableimpactonmidwiferypracticeintheareasofitsmembers.ThebiennialscientificconferencescouldbeperceivedasAMRN'sflagshipactivityandhavebeennotablysuccessful,becomingrenownedinternationally.Around1500midwiveshavebenefitedfromeducationalprogrammesatregionalornationallevel.Theseincluderesearchmethodology,evidence-basedpractice,scientificwritingandcommunicationskills.Attentionneedstobegiventosomeaspectsofgovernanceandorganization,aswellastocommunicationstrategiesincludingwebsitesandnewsletters.TechnicalsupportfromtheKarolinskaInstituteinStockholmhasprovidedgoodcollegiatesupportandpro
