Background

TheBPQYhasimprovedtheSocialSecurityAdministration’s（SSA）effortstoinformSocialSecurityDisabilityInsurance（SSDI）beneficiariesandSupplementalSecurityIncome（SSI）recipientsabouttheirdisabilitybenefitsandtheuseoftheworkincentives.TheBPQYprovidesasnapshotofthebeneficiary'sbenefitsandworkhistoryasitisstoredinSSA'selectronicrecords.

SSA'sEffortsToShare&CorrectInformation

EveryyeartheSocialSecurityStatementissentto140millionworkers.Itlistsaperson'sworkearningsandprovidesinformationforlongrangefinancialplanning.ItalsoensuresthatreportedearningsandotherinformationsuchasnameanddateofbirtharecorrectonSSA’srecords.IndividualsareaskedtoidentifyanyproblemsandcontactSSAtoadjusttheirrecords.TheBPQYsimilarlycommunicatesinformationonSSArecordsspecificallyforbeneficiarieswithdisabilitiesandencouragesplanningforareturntowork.TheinformationisgenerallyaccuratebutifearningswerenotreportedbythebeneficiaryorifworkreportswerenotprocessedbySSA,thedatamustbeupdatedandcorrected.SimilartotheSocialSecurityStatements,everyBPQYwithadiscrepancyshouldbefixedassoonaspossible.

QualityReviewHelpsImproveAccuracy

Communicationaboutadisabilitybeneficiary’sSSAworkrecordbeginswithaBPQY.Accuracydependsonthedatabasesfromwhichtheinformationisdrawn.Everybeneficiaryorcounselororadvocateactingonabeneficiary’sbehalfshouldreviewtheBPQYdataandreportdiscrepanciestoSSA.DiscrepanciesorerrorsinanyitemontheBPQYmustberesolvedassoonaspossibletoavoidmisunderstandingsandoverpayments.

∙SSID（SupplementalSecurityIncomeDisplay）-SSIprogramdata,withacompleterecordofthecashbenefitspaid,scheduledmedicalreviewdates,andotherworkincentivesusedforbothSSIcashbenefitsandMedicaid.TheSSIDrecordprovidesamonthlylistingofestimatedorverifiedearnings.

ItisimportanttonotethattheBPQYisasnapshotintimeandmaynotreflectearningsorotherinformationthathaschangedsincethelasttimetheelectronicrecordwasupdated.AnydiscrepanciesshouldbebroughttotheattentionofSSAassoonaspossible.AlaterBPQYwillconfirmthecorrectionwasmade.

WhoCanGetABPQY?

TheBPQY（SSA-2459）anditscoverletteraregiventobeneficiarieswhorequestit.Withaproperwrittenauthorization,twoReleasesofInformation（seepages14–16）（SSA-3288）tocoverbothSocialSecurityandU.S.InternalRevenueServicerecords,aBPQYcanbegiventoarepresentative,advocateorotherorganization（e.g.,BenefitsPlanning,AssistanceandOutreachorganization）.

BPQYUnavailable-AskforA"BenefitsPlanningInformationRequest"

IfaBPQYisneededforsomeonereceivingdisabilitybenefitsbutSocialSecurityisunabletoproduceone,amanualcertificationformisavailableonpage17ofthisBPQYHandbook.Followthisprocedure:

1.Printtheform"BenefitsPlanningInformationRequest"-page17

2.ThebeneficiaryshouldsigntwoConsentforReleaseofInformationforms,theSSA-3288’sshownonpages14-16,asexamples.

3.SendallthreeformstothelocalSocialSecurityofficewithareturnaddressorfaxnumberforthereply.

HowtogettheBPQY

Beginninginearly2004allSSAofficesnationwideincludingtheagentsatthetollfreenumber800-772-1213canprocessarequestforaBPQY.

IfyouareabeneficiaryinterestedingettingaBPQY,callyourSocialSecurityofficeor800-772-1213andaskforit.ItismailedtoyouraddressasshownonSSA'srecords.AsignedconsentisrequiredonlyiftheBPQYissenttosomeoneotherthanyou,thebeneficiary;yourRepresentativePayeeoryourAuthorizedRepresentative.TwosignedConsentforReleaseofInformation（SSA-3288）formsmustincludetheSocialSecurityNumber（SSN）ortheClaimNumberoftheworkerunderwhoseworkrecordthebenefitsarepaid.（TheClaimNumberappearsonthebeneficiary’sMedicarecard.）

Ifyouareacounselororadvocate,youmusthavethebeneficiarysigntwoConsentforReleaseofInformation（SSA-3288）formsseeabove.
