1.PURPOSE
TheseinstructionsareissuedtoSubcontractorscarryingouttheworkontheProject,itoutlinestheHealth,Safety,EnvironmentalandSecurityrulesandprocedures,whichSubcontractorsarerequiredtofollowwhenundertakingwork.
2.SCOPE
TheinstructionsareproducedprimarilyfortheuseofSubcontractors’managementandsupervisorystaffwhoarerequiredtoensurethattherulesandproceduresarebroughttothenoticeofallSubcontractors’employeesandthatsuchrulesandproceduresarestrictlyfollowed.
IfthereisanydoubtormisunderstandingaboutthecontentsofthisprocedureSubcontractorsshouldconsulttheCONTRACTORSiteSHESManagerforclarification.
WhereSubcontractorsrequirespecialprecautionsormoredetailedguidanceonSHESproceduresheshoulddiscussthiswiththeCONTRACTORSHESstaffordisciplinesupervisors,priortocommencementofhiswork.
3.DEFINITIONS

Subcontractorsandtheirlowertiersubcontractorsengagedtocarryoutworkontheproject.
4.REFERENCEDOCUMENTS
ThedocumentslistedinAttachment-1shouldbereadinconjunctionwiththisinstruction,whicharekeystosuccessfulSHESManagement.
TheCONTRACTORshallproduceadditionalProjectspecificworkpracticesandproceduresastheprojectdevelops.
5.RESPONSIBILITIES
5.1SiteSHESManager
TheCONTRACTORSiteSHESManagershallprepareSHESInstructionsforissuetoSubcontractors.TheCTJVSubcontractsManagershallincludeSHESInstructionstoSubcontractorsinallSubcontractsEnquiries.
5.2SubcontractsManager
TheSubcontractsManagershallensure,inplacingSubcontractsthatallSubcontractorsacceptandagreetoimplementthisdocument.
ReportingtotheSiteManager,theSubcontractsManagerhasthefollowingresponsibilities:
-
∙ToprovideSubcontractorswiththeinformationrequiredinadvanceofcontractaward,withallsiterules,andproceduresrelevanttoSafeWorking.
∙TomakeSubcontractorsawareofknownspecialconditions,includingunusualhazardsorrisksthatmayimpactonSubcontractors’executionoftheirworksincludingHealthandEnvironmentaspects.
∙TomakeSubcontractorsawareofSecurityrequirementsincludingElectronicGateControlSystem.
5.3LegalObligations
SubcontractorsandtheiremployeesshallconforminallaspectsoflegaldutiesandresponsibilitiesaslaiddownbylocalLegislationsandinthecaseofconflictorcontradictionshallallowforcompliancewiththemoststringent.
TheseinstructionsdonotrelieveSubcontractorsfromhislegalorcontractualobligations.
5.4ContractualObligations
Subcontractorsshallensurethatallequipment,plant,machineryandapparatusbroughtontoorusedontheprojectisinasafeconditionandwithoutrisktohealth,safety,ortheenvironmentandismaintainedtoanacceptablestandard.AllequipmentmustbeinspectedbysubcontractortoensureitisinasafeconditionbeforeitisofferedforQGinspectionpriortomobilizationtosite.
Allnecessarytestandexaminationcertificatesmustbeavailableforinspectionatalltimes.
TheCONTRACTORandtheCOMPANYretainstherighttostopanyoperation,activityorerectionofplant/equipment,etc.ifitisconsideredthatthereisahazardtothesafetyandhealthofsitepersonnel(orothers)orthepossibilityofenvironmentalorecologicaldamage.
TheCONTRACTORwillnotacceptanyresponsibilityforanyincreasedcostarisingoutofsuchaction.
Subcontractors’employeesandlowertiersubcontractorsshallobeyanyreasonablewrittenorverbalinstructionsgivenbytheCONTRACTORorinthecaseofanemergencybytheCOMPANYRepresentativeinrespectofhealth,safety,environmentalandsecuritycontrols.
Subcontractorsshallensureallemployeesarecompetentintheirassignedposition,andarefitforworkundertheanticipatedconditionsandarefreefrominfectiousdiseaseasevidencedbymedicalexamination.
5.5Undertaking
SubcontractorRepresentativesmustsignthisundertakingasidentifiedbelow.
ItmustbereturnedtotheCONTRACTORSiteSHESManagerwithacopytoConstructionManager,beforecommencementofwork.
Thisundertakingisonlyvalidforthedurationoftheparticularcontractforwhichitwassigned.
Anysubsequentcontractsrequirearenewaloftheundertaking.
THERESPONSIBILITYFORENSURINGTHATSUBCONTRACTOR’SEMPLOYEES,ANDITSLOWERTIERSUBCONTRACTOR’SEMPLOYEES,UNDERSTANDANDCOMPLYWITHTHERELEVANTSHESPROCEDURESRESTSWITHSUBCONTRACTORHIMSELF.

6.1SHESOfficers

TheSeniorsubcontractorSHESOfficerwillsubmitonaweeklybasisadetailedSHESreportasperAttachment3.
AllSubcontractorSupervisorsandSHESOfficersshallbefluentinEnglish.
TheSubcontractorSHESOfficersshallliaisewiththeCONTRACTORtoensurethatallnecessarySHESprecautionsarebeingadheredtoandthateveryoneconcernedisproperlyfollowinganyinformationorinstructiononhealth,safety,environmentandsecurityissuedbytheCONTRACTOR,theCOMPANYorSubcontractors.
ThisdoesnotremovefromSubcontractors’supervisionandmanagementlevelstheresponsibilitytoensurethattheiroperationsarecarriedoutinasafe,healthy,environmentallyresponsibleandefficientmanner.
TheSHESOfficershallholdarecognizedSHESqualification,andwillbesubjecttoapprovalbytheCONTRACTOR.
SubcontractorsshallincludeintheirSHESPlanthenumberofSHESOfficersheintendstoemployontheprojectovertime.
6.2SHESOrientationCourse&SiteAccess
SubcontractorsshallprovidetheCONTRACTORSecurityManagerwiththefollowingdocumentsforeachemployeeintendedtoworkonsite.
∙PhotocopyofPassportorIdentityCard
∙Photocopyofdrivinglicense,shouldheberequiredtodriveonsite.
SubcontractorsshallensurethatallemployeesattendtheSHESInductioncourseorganizedandimplementedbytheCONTRACTOR.
SubcontractorshallensurethatallworkersaregivenasecondlevelinductioncoursespecifictotheirworksandworkareasaftercompletionoftheCTJVinductioncourse.Recordsofthissecondinductionmustberetainedbysubcontractorforauditpurposes.
6.3MethodStatements
SubcontractorsshallsubmitworkmethodstatementsforallworkinlinewithprocedureQCS/50/00/SA/PC/NA/107.
ThesemustbeissuedtotheCONTRACTORConstructionManageratleast14daysbeforeworkisduetocommence.
Inadditiontoworkassociatedwithpermitstowork,workmethodstatementsshallbesubmittedforthefollowing:
-
∙Installationanddismantlingofscaffolds,
∙WorkonEnergizedElectricalSystems,(hot-linework),
∙Workinsideconfinedspaces,includingrescueoperations,
∙Installationofheavy,tall,orunbalancedequipmentandstructures,usingcrane,ginpole,etc.
∙Heavylifts,(inexcessof20tons)
∙Useofcranehoistedmanbaskets
∙Installationanduseofcableclimbersandgondolas
∙Ionizingradiationwork,
∙Workonoil-filled/gas-inutilities,electricityenergizedequipment,
∙Handlingandstoringoforganicsolventsandtoxicorharmfulchemicals,
∙Deepexcavationforfoundationandtrenching,
∙Blastingactivitiesrequiringtheuseofexplosives,
∙Anyworkinvolvinguniqueorspecializedmachineryorequipmentoperations
∙WorkrequiringtheLockoutandTagoutofequipment.
∙Workinthevicinityoforinsidetheexistingplant(Incl.shutdownandmaintenancework),
∙Anyworkrequiringtheuseofrespiratoryprotection,otherthantheuseofstandarddisposablenuisancedustmasks.
∙PneumaticTesting,AirBlowing,highpressureandchemicalcleaning,and
∙OtherworkdesignatedbytheCONTRACTOR‘sConstructionManagerorwhenrequestedbyCOMPANY.
TheCONTRACTORAreaSuperintendentsand/ortheSiteSHESManager,responsiblesupervisorasappropriate,shallreviewallworkmethodstatements.
6.4RiskAssessment
SubcontractorsarerequiredtoproduceRiskAssessmentsforallworksMethodStatementsgenerated.TheRiskAssessmentistofollowtheformatoutlineinAttachment-3andinprocedureQCS/50/00/SA/PC/NA/107.
Theriskassessmentformidentifies:
-
∙Hazardsrelatingtotheactivity.
∙DetailsofRiskControlmeasuresinplace.
