Parmilesfacteursfavorisantlesconduitesaddictives,certainescaractéristiquesdufonctionnementfamilialsontsouventévoquéesdanslestroublesalimentairesetdanslesdépendancesauxsubstancespsychoactivesouàl’alcool.Lamiseàl’épreuvedesobservationsdesprocessusfamiliauxpardesrecherchesempiriquesintégrantdifférentsniveauxépistémologiquesetméthodologiquesaconduitàdesrésultatscontradictoiresconcernantlesdifférentesformesdefonctionnementsfamiliauxassociésàcespathologiesaddictives.Cetarticleproposeunerevuecritiquedelalittératuredanslecadredesconceptionssystémiquesconcernantlesdérèglementsfamiliauximpliquésdansl’apparitiondestroublesalimentairesetdesdépendancesauxsubstancesouàl’alcoolchezlesadolescents.Ils’agiraégalementdediscuterlasimilaritédescaractéristiquesfamilialesentrecespathologiesaddictivesetl’hypothèsed’unetypologiefamilialepropreauxconduitesaddictives.

Abstract

Amongfactorsfavoringtheaddictivebehaviors,somespecificfamilyprocessesareoftenidentifiedineatingdisordersanddependenceswithpsychoactivesubstances.Differentmodelsofsystemicorientationdescribedthefamilialdisturbancesinvolvedintheaddictivepathologiesemphasizingeithertheemotionalinteractionandtheprocessesofdifferentiationfamily,eitherthestructureandtheleveloffamilycohesion.Thisarticleproposesacriticalreviewofthesystemicliteratureonthefamilialdisturbancesinvolvedinthefamiliesofadolescentswitheatingdisordersorwithasubstance-dependence.Thesystemicmodels（Bowen,Minuchin）arereferencevaluesintheunderstandingoffamilyprocessesandtherapy

withfamilies.Theobservationsofthesefamilyprocesseshavebeenconfrontedtoempiricalresearchesintegratingdifferentepistemologicallevelsanddifferentmethods.Theempiricalresultsareheterogenousconcerningthedifferentformsoffamilyfunctioningassociatedwiththeseaddictivepathologies.Theempiricalstudiessupportthehypothesisofafamilydysfunctioninginthedifferentaddictivebehaviorsanddemonstratetherelationshipbetweentheseverityoffamilialdisordersandtheseverityoftheaddictivedisorders.However,thefamilyconfigurationdescribedbythetypologicalmodelsisrarelyfoundcompleteorwithasimilarintensitybetweenthefamilies.Thestudiessupportthehypothesisofanemotionaldistanceinthefamilyinteractionsandalackofparentalcarewhichcouldcorrespondtofactorsofchronicityorvulnerability.Comparingthefamilycharacteristicsofthesetwotypesofaddictivebehaviors,theliteraturesupportstheexistenceof

（1）somespecificfamilycharacteristicsaccordingtothetypeofaddictivebehavior（inparticulartheconflictavoidancewhichappearsspecificallyinanorexicfamilies）

（2）somesimilarfamilypatternsofinteractionsbetweenthemembersofaddictivefamiliesinparticulartherepresentationofalowfamilycohesion,anintergenerationalenmeschmentandahighlevelofcontrol.Thesefamilypatternssuggestrathertoadimensionalapproachofthefamilycharacterizedbydifferentprotectiveorriskfactors.Wesuggestthatthesefamilypatternscouldconstitute,accordingtotheirimportance（intensityandfrequence）,afamilyriskfactorofdevelopmentoraggravationoftheaddictivebehaviors.

Abstract

Background

Althoughincidenceofschizophreniaishigheramongcannabisusersandmarijuanaisthemostcommonabuseddrug

byadolescents,etiologicallinkagebetweenschizophreniaandcannabisuseisstillnotclarified.Clinicalexperiencessuggestthatregularcannabisusercanshowsimilarpsychoticepisodetoschizophrenicdisordersbutitisstillunclearifchroniccannabisusewithschizophreniformdisorderisadistinctentityrequiringspecial

therapy

oritcanbetreatedasclassicalschizophrenia.Therearenodataavailableonthecomparisonofpharmacotherapybetweenschizophreniformpatientswithandwithoutcannabisuse.

Methods

Clinicaldataof85patientswithschizophreniaspectrumdisorderwereanalyzedretrospectively.Cannabisusewasnotreportedby43persons（Cnbs0subgroup）and42patientsusedregularlycannabisduringatleast1 year（Cnbs1subgroup）.Comparisonofanamnesis,familyhistory,social-demographiccondition,positiveandnegativesymptoms,acuteandlong-term

therapies

recordedbyclinicalinterviewswasperformedwithchisquaretests,logisticbinaryregressionandt-testsusingSPSS13.0forWindowssoftware.

Results

Menwereover-representedincannabisdependentgroupwhilemeanagewasloweramongthemcomparedtoCnbs0subgroup.Prevalenceofsuicidalattemptwasincreasedinmenwithoutcannabisuse（OR = 5.25,p = 0.016）.Patientswithoutcannabisusespentmoretimeinhospital（p = 0.026）andsmokingwasmorefrequentamongthem（OR = 1.36,p = 0.047）.Thechancetogetolanzapineforacute

therapy

andaripiprazolforlongterm

therapy

wasmorethantwofoldinCnbs1subgroup（OR = 2.66,OR = 3.67,respectively）.However,aripiprazolwasusedforacute

therapy

withsignificantlylowerriskinCnbs1subgroup（OR = 0.47,p = 0.023）.Olanzapinewasadministeredforlongterm

therapy

inahigherdosetoCnbs0patients（p = 0.040）.AlsohigherdoseofrisperidonLAIwasusedinwomenwithoutcannabisdependencycomparedtowomenofCnbs1subgroup（p = 0.020）.Positiveandnegativesymptomsandfamilyhistorydidnotdiffersignificantlybetweenthetwosubgroups.
