附录1应急响应审核单
1.PURPOSE
目的
ThepurposeofthisprocedureistoprovideasafesystemofworkbytheimplementationofemergencyresponsemeasuresandtoidentifytheresponsibilitiesandlegalrequirementsatworksiteswhereProjecthastheprevailinginfluence.
本程序的目的是通过执行应急反应措施为工作提供一个安全体系，明确在项目工地的责任和法律要求。
2.SCOPE
范围
2.1ThisproceduredescribestheactionstobetakenbykeypersonnelintheeventofanemergencysituationarisingonProjectSiteandoffsiteconstructionactivitiesassociatedwiththisproject.
本程序说明在项目现场及与项目有关的场外施工活动中出现紧急状况时，关键人员可以采取的措施（行动）。
2.2Theprocedurealsocatersforemergenciesarisingoutofofficehours.Itisanticipatedthatnotificationofemergenciesoccurringduringofficehourswillbevianormalchannels.Theresponsearrangementswillhowever,beaspertheprocedure.
本程序还考虑办公时间以外出现的紧急事件。
在办公时间出现的紧急事件将由正常的渠道通知，响应安排将按本程序执行。
3.LEVELSOFRESPONSE
反应等级
3.1Asemergencysituationsdifferinseverity,forthepurposeofestablishingtheappropriatelevelofresponserequired,incidentsarerankedasLevels1,2and3.
由于紧急事件发生的严重等级各不相同，为了满足适当的响应级别的要求，将事故等级分为1，2和3级。
Level1-Thiswouldbe,butmaynotbelimitedto,amedicalrescue,whichistheresultofillnessorminorinjuryorminorincident.Someexampleswouldbe:
1级---包含疾病、轻微的伤害或事故导致的医疗救护在内的轻微事件。
例如：
a）Anenvironmentalspillof1barrelorless.
1只桶或不到1只桶（的化学品）溢溅至环境中。
b）Afire,thatwasextinguishedwithahandheldfireextinguisher.
可以用手提式灭火器扑灭的火灾。
c）Asinglevehicleaccidentwithoutinjury
没有人员伤害的单独的车辆事故。

Insuchinstancestheresponsewouldbelimitedtotheimmediatesupervisor.
在这类事例中，响应（动作）应局限于直接的主管。
Level2-Thiswouldbeamedicalrescue,whichistheresultofaninjury,causedbyanaccident,seriousenoughtorequireinvestigationandwhichmayhavebeenreportedtothelocalauthoritybytheContractor/Subcontractor.Someexampleswouldbe:
2级-这将是一种医疗救助，是由于事故造成的伤害，它严重到足以要求调查（原因），它也可能已由承包商向当地有关部门报告，例如：
a）AnenvironmentalspillgreaterthanIbarrelbutlessthantenbarrels.

Level3-Thisisanemergencysituation,whichhasresultedinthemobilizationofthesiteemergencyresponseorganization.InsuchinstancesthefullProjectEmergencyResponseTeamwillbemobilized.Someexampleswouldbe:
3级-这种紧急事件会导致现场紧急事件响应组织的介入。
此时，整个项目紧急事件响应小组都进入现场，例如：
a）Amajorenvironmentalspill
重大的环境溢溅（事件）
b）Amajorfire.
重大火灾
c）Avehicleaccidentresultinginfatalitiesormajorinjuries.
导致死亡和严重伤害的车辆事故
3.2ThePrimaryEmergencyResponseTeamisDescribedBelow:
初级紧急事故响应小组如下所述：
a）ProjectManager（EmergencyTeamLeader）
项目经理（紧急小组领导人）
b）ConstructionManager（AlternateEmergencyTeamLeader）
施工经理（紧急小组候补领导人）
c）Contractor/SubcontractorHSEManager
承包商/分包商HSE经理
d）Construction/ProjectHSEManager
项目HSE经理
3.3AsufficientnumbersofpersonswillreceiveEmergencyResponseTrainingtoensurethatthereisadequetcoverageduringabsenceperiods.
要有足够的人员经过紧急事故响应培训，保证有人缺席时有足够的人员可替代。
Level2andaboveemergencyrespondmustreporttoownertimelyandownerwillattendinvestigation
二级和二级以上应急响应必须报告给业主，业主将参加调查
4.ACTIONS/REQUIREMENTS

4.1General
通则
4.1.1InorderthattheEmergencyResponseTeamcanfunctionproperlythefollowingwillbeprovided:
为使紧急响应小组能正确发挥作用，必须提供如下（条件）：
a）Alistingofallpersonnelemployedandtheirworklocations.
所有雇员的名单，及他们的工作场所。
b）Listofemployees’homeaddressesandtelephonecontactnumbers
雇员家庭地址及联系电话号码清单.
c）Up-to-daterosterlistofEmergencyResponsePersonnelandmanagementpersonnel.
紧急响应人员和管理人员的最新名册。
d）TelephonenumbersofEmergencyResponsePersonnel.

e）Contractors’/Subcontractoremergencycontactnumbers.
承包商/分包商的紧急联系（电话）号码。
f）TheallocationofanEmergencyResponseCenter.

4.1.1TheConstructionManagementwillcarryamobilephoneatalltimeswhileondutyorwhenoncall.
值班工程师在其当班时将随时身带移动电话和呼机。
4.1.2OutofofficehoursatelephoneansweringmachinemaybeactivatedtoprovidecallerswithProjectout-of-hoursemergencytelephonenumbers.
下班后，应使电话应答机处于工作状态，以向来电者提供项目下班时用的紧急电话号码。
4.1.3TheidentifiedconferenceroomatProjectwillalsoserveastheEmergencyCo-ordinationRoom,toprovideabasefromwhichtheEmergencyResponseTeamcanco-ordinateeffortstohelpresolveanyarisingcrisis.
项目的指定会议室也将用作紧急事故协调室，成为紧急响应小组的指挥中心，进行协调作业，协助解决出现的危机。
4.2ResponsetoEmergencies
对紧急事故的响应
4.2.1Level1
1级
4.2.1.1TheimmediatesupervisorhastheresponsibilitytocontacttheContractors’HSEManager/Engineertoprovidefullparticularsoftheemergency.TheContractors’HSEManager/Engineershallthentakeappropriateactions.
直接的主管有责任与承包商HSE经理/工程师联系，向（其）提供紧急事件的全部详情。
然后承包商的HSE经理/工程师必须采取适当行动。
4.2.1.2IfforanyreasontheContractors’HSEManager/EngineercannotbecontactedthecallshouldbedirectedtotheAreaConstructionManagerordesignate,whocaneitherdealwiththematterdirectlyorcontactothersuitablepersonnel.
如果由于种种原因，联系不上承包商的HSE经理/工程师，应直接打电话给区域施工经理或其指定人员，他可以处理这种事情或者能联系上其它合适人员。
4.2.2Level2
2级
4.2.2.1AboveactionswillalsoapplytoLevel2emergenciesbutintheeventofamoreseriousaccidentorincident,Project/ConstructionHSEManager,orhisdeputy,andtheAreaConstructionManagermustbeinformed.Project/ConstructionHSEManagershallcommenceincidentinvestigationactivities.
对这一级的紧急事件也要采取上述行动，但若事故较为严重，还应通知项目HSE经理或其副手、区域施工经理。
项目HSE经理必须开始事故调查活动。
4.2.3Level3
3级
4.2.3.1Allcallsconcerningalevel3emergencyshouldbemadetotheProjectManager.OnreceiptofacallindicatingthataLevel3Emergencyhasarisenthefollowingactionsshallbetaken.ProjectManagershallimmediatelycontactallEmergencyResponseTeamMembersadvisingthemofthesituationandinstructingthemtoproceedtothesiteoftheemergencyandifnecessarytomobilizetheEmergencyCo-ordinationRoom.OncecontacthasbeenmadetheProjectManagershouldimmediatelyproceedtothesceneoftheemergency.
凡涉及3级的紧急事件均应打电话给项目经理。
一旦收到出现3级的紧急事件的电话，应采取下列行动。
项目经理应立即与全体紧急响应小组成员联系，告诉他们发生的情况，并指示他们去事故现场，必要时将紧急事故协调室搬过去。
一旦联系完毕，项目经理应立即去事故现场。
4.2.3.2IftheEmergencyCo-ordinationRoomhasbeenmobilizedtheProjectManagershalladvisetheContractors’HSEManagerthattheEmergencyCo-ordinationRoomhasbeenmobilizedandthatallcommunicationsshouldbebetweenthetwocenters.
一旦紧急协调室已经搬迁，项目经理应通知承包商HSE经理紧急协调室已完全搬好，所有的通信可在两个中心之间进行。
4.2.4Oncethenatureoftheemergencyhasbeenestablished,theProjectManagermustdeterminetheextentofback-upandsupportrequired.HemustalsoliaisewiththeContractors’HSEManagerandestablishwhetheraProjectRepresentativeisrequiredattheContractors’premises,PoliceHeadquarters,Hospital,etc.
一旦紧急事件的性质已经确定，项目经理必须决定后援和支持的程度（范围）。
他还必须与承包商HSE经理保持联系，以明确项目高级代表是在承包商办公室、警察总部还是在医院等（为好）。
4.2.5TheProjectManagermayrequiretheContractortoprovideaprintoutoftheup-to-date"Personsonsite"list,fortheaffectedlocation.
项目经理可要求承包商提供一份受害地点更新的“现场人员”名单。
4.2.6Anyoneinvolvedinanemergencyresponseshallmaintainalogofallinformation,（issuedandreceived）andofallactionstaken.Forthispurposeclearandlegiblenotesonplainpaperareacceptable.Attheconclusionoftheemergency,thenotesmustbecollatedan
