2.1ProjectDescription项目描述
SECCOandtheirContractorsaredevelopingtheShanghaiEthyleneCrackerComplex.ThePolyethyleneprojectispartofthiscomplexandhasbeenawardedtotheconsortiumTECHNIP-COFLEXIPEngineeringConsultantCo.,Ltd./SinopecShanghaiEngineeringCompanyasanEPCcontract.
赛科与其承包商正合力组建上海乙烯裂解联合体。
德希尼布工程咨询有限公司/中石化上海工程公司将作为EPC承包商负责其中的聚乙烯项目。
2.2PurposeandScope目标与范围
TheHSEplancoverstheengineering,procurement,constructionandpre-commissioningfortheproject.Thisplandescribeshowthehealth,safetyandenvironmentalmanagementsystemwillbeimplementedontheproject,incompliancewiththecontractandthedocument1821V00SP620001Rev.2,includingtheSECCOStandardsandProcedures.TheHSEmanagerisresponsibleforthepreparationofthisHSEplan.TheProjectmanagershallreviewandtheProjectdirectorapprovesthisdocument.TheHSEplanshallbereviewedandifrequiredupdatedatleastonceeveryyear.
HSE计划将贯彻于项目中工程，采购，施工与试车各个阶段，并且依据合同，
文件1821V00SP620001Rev.2与SECCO标准与程序对项目中HSE管理系统的执行进行说明。
HSE经理负责整个HSE计划的撰写，项目经理进行评估后由项目总监批准。
计划应定期进行审核并在必需的情况下每年至少更新一次。
2.3ProjectHSEPolicy项目HSE方针
TheProjectHSEpolicyissignedbyProjectdirectoranddisplayedatprominentlocationsthroughouttheproject.AsignedcopyinEnglishandChineseisattached（Appendix1）.Thestatementshallbeupdatedasaresultof:
HSE方针在整个项目进行过程中占据着极为重要的地位，并由项目总监签字生效。
附录1中附有已签署的中英文版HSE方针。
方针中说明应随着下列情形的出现而变更：
ØChangesintheProjectmanagementstructure.
项目管理结构的变更。
ØVariationstotheprincipalscopeofworkorchangesinlegislationorindustrybestpractices.
基本工作范围发生变化或法规和工业最佳细则有所变更。
ØIncidentinvestigationsandfindings.
事件调查与结果。
ØRecommendationsarisingfromaudits,inspectionsorotherinvestigativeprocesses.
在审核，检查或其它调查过程中听取的建议。
2.3.1HSEPrinciplesHSE原则
TheHSEprinciplestobefollowedontheProjectare:
在项目中应遵循下列HSE原则：
ØCarefulplanningandapplicationofHSEstandardscanpreventallaccidents.
周密的计划和HSE标准的贯彻能预防一切事故。
ØThereshallbevisiblemanagementcommitmenttoHSEandcontinuousimprovement.
具有明确的HSE管理承诺并不断进行改善。
ØHSEistheresponsibilityoflinemanagement.
各层管理人员都应对HSE负责。
ØHSEobjectivesandgoalsshallbesetandshallbespecific,measurable,achievableandrealistic.
制定确切可行的HSE目标。
ØHSEstandardsshallbeavailableandbeeasilyunderstood.
HSE标准应实用易懂。
ØAuditsofHSEstandardsshallberegularlyperformed.
定期执行HSE标准的审核。
ØTheworkforceandstaffshallbecompetenttoperformtheirallocatedtasksandfunctions.
工人与全体人员都应胜任所给任务与职责。
ØEmployeesareentitledtoreceiveHSEinformationabouthazardsandrisks.
员工应获得相关风险的HSE信息。
ØEmployeesshallbetrainedtosafelyperformtheirwork.
对员工进行培训使之能安全工作。
ØHSEinductionismandatoryforallworkerspriortoresumptionofduty.
在工作开始之前，所有工人都必须接受HSE归纳课程。
ØEffectiveHSEmeetingsandtrainingshallbeintegraltotheProject.
有效的HSE会议与培训是项目一部分。
ØEffectivemotivationandanopencommunicationstructurewithinwhichdiscussionofanyHSEmatterscanfreelytakeplace.
实施有效的激励系统，建立一个开放的沟通结构体系使HSE相关事宜的讨论能随时随地进行。
2.3.2HSEObjectivesHSE目标
TheseshallbemeasuredbyauditthroughouttheProjectlifetime.TheHSEobjectivesare:

HSE目标为：
ØPreventallincidentswithpotentialtoinjurepersonnelorcausedamage/loss.
采取措施预防一切可能造成人员伤害或损失的事件。
ØEnsureasafeworkingenvironmentforallpersonnel.
确保提供给所有人员一个安全工作环境。
ØAnyunsafeactorconditionobservedbyanyworkershallbereportedimmediately.
任何工人一旦发现危险行为或情况，应立即向相关部门报告。
ØEnsureahealthyworkingenvironmentforallpersonnel.
确保提供给所有人员一个健康工作环境。
ØMinimizeexposuretopotentialhealthhazardsarisingfromthework.
最小化工作中接触到的健康隐患。
ØPreventallincidentsthatcoulddamagethereputationoftheconsortium.
防止一切有损联合体声誉的事件。
ØMaintaincloserelationswiththelocalcommunitytoimproveourreputation.
与当地社团保持紧密联系以提高我们的声誉。

2.4HSEOrganizationandResponsibilitiesHSE组织与职责
TheprojectHSEorganizationchartisattachedinAppendix2.TheHSEcomplianceisalinefunction,managedthroughboththefunctionalandtechnicalgroups.AlloperationsmanagersaredirectlyresponsibleforHSEcompliancewithintheirscopeofoperationsandareassistedbyHSEstaffthatprovidesadvicenecessaryformanagerstocomplywithbothregulationsandourpolicy.
TheHSEprojectleadershipandresponsibilitiesaredescribedhereafter.
附录3中附有项目HSE组织结构图。
通过职能与技术团队形成从上到下统一的HSE管理。
所有操作经理在工作范围内直接负责确定其操作与HSE相符，并由HSE人员协助提出必要建议以贯彻其法规和方针。
下面详细说明了HSE项目领导层及其职责。
2.4.1ProjectDirector项目总监
TheProjectdirectorisresponsibleforthehealthandsafetyoftheworkforceandfortheprotectionoftheenvironmentwithinthescopeofalloperationscarriedoutontheproject.
Heisalsoresponsibleforthefollowing:
项目总监将负责全体工人的健康与安全以及各项活动中对环境的保护，同时下列也属于其职责范围：
ØApprovetheHSEprogram.
批准HSE程序。
ØAllocatesufficientresourcesforthemanagementoftheHSEprogram.
给予充足物力支持HSE程序管理。
ØImplementationoftheHSEmanagementsystemwithintheproject.
项目上HSE管理系统的实施。
2.4.2ProjectManager项目经理
TheProjectManagershallberesponsibleforthefollowing:
项目经理应负责：
ØEnsurethatthedesignhasincludedconsiderationsforsafetyduringconstruction,commissioning,maintenanceandoperations.
确保在施工，试车，维护与操作各阶段中安全都包括在计划之内。
ØEnsurethatmeasuresareinplacetocontrolsecurityonsite.
确保采取合理的措施控制现场安全。
ØEnsurethatcommunicationsonsiteareadequatetoadviseallpartiesofrisksorconcernsastheyareidentified.
确保现场具有完善的通讯以便能及时向所有人员传达已识别的风险或利害关系。
ØEnsurethatarrangementsforthecoordinationofallpartiesareinplacetopreventinterfacerisks.
确保合理安排好各部门之间的协作以避免人为造成的风险。
ØContributeasnecessarytotheHSEprogram.
对HSE程序提出一些重要的建议。
ØEnsurethattherequirementsoftheDesignSafetyplanaspertinenttoconstructionactivitiesintermsofriskassessment,workmethodstatements,environmentalconsiderations,constructabilityetc.areknownandreferencedinthisplan.
确保对施工中安全计划设计的要求有所了解并以此计划为参考，如风险评估,工
作方法，环境考虑及可建筑性等等。
ØEnsurethatlinepersonneltakeownershipoftheHSEprogram.
确保各层人员都充分了解HSE程序。
2.4.3ProjectHSEManager项目HSE经理
ReportstotheProjectDirectorandisresponsible:
向项目总监汇报，职责为：
ØToprovideprofessionaldirectionandguidancetotheproject.
为项目提供专业的方向与指导。
ØTotakeownershipoftheinitialdevelopmentandformulationoftheHSEplan.
负责HSE计划最初发展与确定。
ØToensurethattheprojectreceivestheHSEserviceitrequiresandisawareoftheprojectmanagement’sevaluation.
为确保HSE工作贯彻到项目中要求对此进行管理评估。
ØToassignHSEpersonneltotheprojectsuitablyqualifiedbyeducation,trainingandexperienceinthetypeofworkinvolvedinthisprojectandexperiencedintheimplementationofthemanagementsystem.
指派的HSE人员应取得相应教育资格，在项目中已参加自己所从事具体工作的相当经验，
熟知管理系统的执行。
ØTofurnishthesitewithHSEproceduresandtechnicalandmanagerialHSEmanualsappropriatetotheproject.
根据项目给现场提供适当的HSE程序与HSE技术管理指南。
ØToinspect,auditandreportontheprojectHSEprogramandHSEperformance.
根据项目HSE程序与实施来进行检查，审核与报告。
2.4.4LeadSafetyEngineer/ProcessEngineer首席安全工程师/工艺工程师
