sClinicalGuidelinesSubcommittee,ClinicalAffairsCoreCommittee,andcosponsoringassociations,anditwaspostedonTheEndocrineSocietywebsiteformemberreview.Ateachstageofreview,theTaskForcereceivedwrittencommentsandincorporatedneededchanges.Conclusions:
ConsideringthatvitaminDdeficiencyisverycommoninallagegroupsandthatfewfoodscontainvitaminD,theTaskForcerecommendedsupplementationatsuggesteddailyintakeandtolerableupperlimitlevels,dependingonageandclinicalcircumstances.TheTaskForcealsosuggestedthemeasurementofserum25-hydroxyvitaminDlevelbyareliableassayastheinitialdiagnostictestinpatientsatriskfordeficiency.TreatmentwitheithervitaminD
(2)orvitaminD(3)wasrecommendedfordeficientpatients.Atthepresenttime,thereisnotsufficientevidencetorecommendscreeningindividualswhoarenotatriskfordeficiencyortoprescribevitaminDtoattainthenoncalcemicbenefitforcardiovascularprotection.

Theconsensusalgorithmforthemedicalmanagementoftype2diabeteswaspublishedinAugust2006withtheexpectationthatitwouldbeupdated,basedontheavailabilityofnewinterventionsandnewevidencetoestablishtheirclinicalrole.Theauthorscontinuetoendorsetheprinciplesusedtodevelopthealgorithmanditsmajorfeatures.Wearesensitivetotherisksofchangingthealgorithmcavalierlyortoofrequently,withoutcompellingnewinformation.AnupdatetotheconsensusalgorithmpublishedinJanuary2008specificallyaddressedsafetyissuessurroundingthethiazolidinediones.Inthisrevision,wefocusonthenewclassesofmedicationsthatnowhavemoreclinicaldataandexperience.
Theepidemicoftype2diabetesandtherecognitionthatachievingspecificglycemicgoalscansubstantiallyreducemorbidityhavemadetheeffectivetreatmentofhyperglycemiaatoppriority(1–3).Whilethemanagementofhyperglycemia,thehallmarkmetabolicabnormalityassociatedwithtype2diabetes,hashistoricallytakencenterstageinthetreatmentofdiabetes,therapiesdirectedatothercoincidentfeatures,suchasdyslipidemia,hypertension,hypercoagulability,obesity,andinsulinresistance,havealsobeenamajorfocusofresearchandtherapy.Maintainingglycemiclevelsasclosetothenondiabeticrangeaspossiblehasbeendemonstratedtohaveapowerfulbeneficialeffectondiabetes-specificmicrovascularcomplications,includingretinopathy,nephropathy,andneuropathy,inthesettingoftype1diabetes(4,5);
intype2diabetes,moreintensivetreatmentstrategieshavelikewisebeendemonstratedtoreducemicrovascularcomplications(6–8).IntensiveglycemicmanagementresultinginlowerA1Clevelshasalsobeenshowntohaveabeneficialeffectoncardiovasculardisease(CVD)complicationsintype1diabetes(9,10);
however,currentstudieshavefailedtodemonstrateabeneficialeffectofintensivediabetestherapyonCVDintype2diabetes
