EarlierstudieshaveshownthatcirculatingghrelinlevelisincreasedinunderweightpatientswithCHF,lungcancer,andlivercirrhosis.Inthepresentstudy,however,plasmaghrelinlevelwasdecreaseddespiteasignificantweightlossinCOPD.Inaddition,earlierstudieshavereportedthatcirculatingghrelincorrelatedpositivelywithBMIinpatientswithCHFandlungcancer.However,thepresentstudydemonstratedthatplasmaghrelinlevelcorrelatedpositivelywithBMIinCOPDpatients.Thus,thereareconsiderablediscrepanciesbetweenthepresentstudyandearlierstudies.Thesediscrepanciesshouldbediscussedindetail.TheauthoralsostatedtheregulationofghrelinsecretionwasdisturbedinCOPDpatients.However,theydidnotclarifythismechanism.

WestatedthattheroleofghrelininpatientswithCOPDmaybedifferentfromitsroleinCHF,cancerandlivercirrhosisanddiscussedthisdifferenceinthelastparagraphofpage9.
