6.MinimumSafetyManningCertificate

ThisistocertifythattheshipwillbeconsideredtobesafelymannedprovidedthatshehasnotlessthannumbersandgradesofpersonnelshowninthisCertificate.

Note:

1）SailorshallholdtheCertificateofCompetencyforratingformingpartofanavigationalwatch.MotormanshallholdtheCertificateofCompetencyforratingformingpartofaengineeringwatch.

2）ThenumberofpersonnelholdingtheCertificateforProficiencyinSurvivalCraftandRescueBoatshallnotbelessthanthenumberofsuchcraftandboatsonboardincaseofpassengership.

3）ThenumbersandgradesofpersonnelgiveninthisCertificatearetheminimummanningrequirementnecessaryfortheship’ssafeoperationanddonotprecludethecarriageofadditionalpersonnelforthepurposeofship’smaintenance，livelihoodservice，cargohandling，laborprotection，etc.However，thetotalnumberofthepersonnelonboardcannotexceedthelimitationofthelife-savingappliances.

7.ShipSafetyNavigationCertificate

3）ThiscertificateisvaliduntilsubjecttoannualsurveysinaccordancewithArticle14

（1）（c）oftheConvention.

9.Ship’sSanitaryCertificate

InaccordancewiththeFrontierHealthandQuarantineLawofthePeople’sRepublicofChinaanditsspecificrulesandotherhealthregulationsconcerned.Thesanitaryconditionoftheaboveshipisconsideredsatisfactoryasaresultofinspection.Thecertificateisherebygranted.

10.DerattingCertificate/DerattingExemptionCertificate

Thiscertificaterecordstheinspectionandderatting/derattingexemptionatthisportandontheabovedateoftheshipundertheflagofnettonnage.Atthetimeofinspection/deratting，theholdswereladenwith

tonsofcargo.

Recommendation:

Inthecaseofexemption，statethemeasurestakenformaintainingtheshipinsuchaconditionthatitisfreeofrodentsandtheplaguevector.

11.InternationalOilPollutionPreventionCertificate

2）Thatthesurveyshowedthatthestructure，equipment，system，fittings，arrangementandmaterialoftheshipandtheconditionthereofareinallrespectssatisfactoryandthattheshipcomplieswiththeapplicablerequirementsofAnnexⅠoftheConvention.

3）ThiscertificateisvaliduntilsubjecttotheperiodicalsurveysinaccordancewithRegulation4ofAnnexⅠoftheConvention.

12.ShipGasFreeCertificate

Thisistocertifythatfollowingaccessiblecompartmentsorspacesofthesaidshiphavebeeninspectedandtested.

Theabove-mentionedcompartmentsandspaceshavebeenfoundfreefromcombustiblegas.

Explanations:

1）ThisCertificatedoesnotincludeallkindsofpiping，pumpandotherequipment.

2）ThisCertificatewouldnotremainvalid，shouldthecombustiblegasesorinflammablematerialsenterthetestedspacesduetothechangeofballast，openingofpipingandvalvesorothercauses.

3）Beforethehot-workiscommencedandwhileitisproceedingtheworkareasincludingtheheat–influencedadjacentspacesaretoberetestedinduetimebycompetentpersonsoftheworkingunit.

（1）（a）（i）to（Ⅳ）ofAnnexⅣoftheConvention.Apipelineforthedischargeofsewagetoareceptionfacility，fittedwithastandardshoreconnection.

2）ThattheshiphasbeensurveyedinaccordancewithRegulation3ofAnnexⅣoftheConventionandthesurveyshowsthattheequipmentoftheshipandtheconditionthereofareinallrespectssatisfactoryandtheshipcomplieswiththeapplicablerequirementsofAnnexⅣoftheConvention.
