EXPLANATORYNOTESONCOMPLETINGTHECHECKLIST

1.ItistheManagementCompanyandtheMaster’sresponsibilitytoensurethataHongKongregisteredshipisoperatedsafelywithoutmarinepollution,andisimplementedasafetymanagementsystemeffectively.Thisself-assessmentchecklistprovidesaframeworkforhelpingtheManagementCompany,theMasterandtheChiefEngineerinassessingthephysicalconditionofaHongKongregisteredship,understandinghoweffectivethesafetymanagementsystemisbeingimplementedonboard.ThischecklistisnotexhaustiveandisonlyageneralguidancesuggestingwhatitemstobecheckedorreviewedbytheManagementCompany,theMasterandtheChiefEngineer.

2.WheneverashipistoenteraUSorAustralianport,thechecklistmustbecompletedandsignedbytheship’sMaster.ItmustalsobeendorsedbytheDPAoftheManagementCompanyandreturnedtoCargoShipsSafetySectionoftheMarineDepartmentbeforeenteringtheUSorAustralianportforeachvoyagetotheUSAorAustralia.ForavesselthathasnoPSCdetentionrecordwithintheprevious12monthstheintervalforsubmissionbetweensuccessivechecklistsshouldnotbemorethanthree（3）months.HoweverforavesselwithPSCdetentionrecordwithintheprevious12monthstheintervalforsubmissionbetweensuccessivechecklistsshouldnotbemorethanone

（1）month.

3.Ifanyoneoftheitemsinthechecklistis‘NO’,itimpliesthatthesafety,pollutionpreventionorsecurityoftheshipisadverselyaffected.Theship’sMastershallrecordthedetailsoftheirregularitiesandproposethecorrectiveactionsaccordingly.Ifthereisdoubtordifficultyinrectifyingtheirregularities,theMastershouldconsultandinformtheManagementCompanyimmediately.

4.TheManagementCompanyand/ortheDPAshallgiveanexecutivesummaryandcommentontheoverallconditionoftheshipanddeficienciesdetected.Inaddition,measuretopreventPSCdetentionandacorrectiveactionplanshallalsobeincluded.

5.Shouldanypotentialproblemsbeidentifiedorfailureofanyequipmentormachinerythatcannotberectifiedbeforeenteringanyport,theship’sMasterortheManagementCompanyshouldpromptlynotifytheportauthority,theflagAdministrationandtheclassificationsociety,asappropriate,inadvancetoavoiddetention.Inthemeantime,theship’sMasterandtheManagementCompanyhavetoendeavourtorectifyalldeficienciesdetectedbytakingappropriatecorrectiveactionsortemporaryrepairs.Theship’sMastershallalsoenterthefactintotheship’slogbook.

6.Thechecklistshouldbekeptbytheship’sMasterfornecessaryfollow-upactionandfuturereference.

7.Aftercompletingthechecklist,theship’sMastershallsendacopyofthechecklisttotheManagementCompanyforvettingandfurthercomment.TheManagementCompanyshallsendthecompletedchecklistwiththerelevantsupportingdocuments（ifany）to:
