OnFebruary19,1985,aB-747SP,flownbyaChinaAirlinesCapt.,sufferedanenginefailurewhilecruisingat41,000ft.TheCapt.leftitonautopilottoolong.Theautopilottriedtomaintainthataltitude,whichwasimpossibleatthatweight,withonly3enginesfunctioning.Asitapproachedthestall,becausethespeedkeptdecelerating,theCapt.finallydisconnectedtheautopilot.Hewasnotprepared,becausehehadfailedtotriminruddertocompensatefortheasymmetricalthrustcondition;

theautopilotwasmaintainingwingslevelbytheuseofaileronandspoilersonly.Thishappenedbecauseautopilotsnormallydonotcontroltherudderinclimb,cruise,ordescent.Theyuseonlytheailerons,spoilers,elevatorsandhorizontalstabtrim.Whenhehitthatdisconnectswitch,theplanerolledrapidlyandenteredadive.Althoughtheplaneexceededthespeedofsound,tearingpartsoffandcausingmajorstructuraldamage,theCapt.wasabletomakearecoveryatafewthousandfeetoverthePacificOcean,afterhebrokeoutofthecloudsandcouldseehisattitudeviaoutsidevisualreference.Therewere,incredibly,onlytwoseriousinjuriestothe274passengersandcrew.
