建设工程竣工消防验收要求

一、验收形式：

建设单位应当组织设计、施工、监理等单位进行竣工消防验收，依据国家和本市工程建设消防技术标准、消防部门审核意见，重点对建设工程中涉及本报告所列各单项中的子项进行核查自验，并做出单项验收、综合评定的结论，填写本报告，方可提请公安机关消防机构验收或者备案。

二、过程控制：

建设、设计、施工、监理等单位，应当按照消防法规、建设工程质量管理法规、国家和本市工程建设消防技术标准，完善施工质量过程控制体系，落实施工组织、设计变更、施工工艺、材料检验、档案管理等的各项制度。

三、单项验收：

重点核查单项中各子项，“符合要求”是指符合国家和本市工程建设消防技术标准、消防部门审核意见、审查通过的设计图纸，按图施工、设施调试开通，功能和运行参数达到设计要求。

四、综合评定：

依据过程控制和单项验收情况，重点核查施工质量控制体系、竣工图纸、质量合格文件、竣工验收原始文件、隐蔽工程验收资料、设备运行调试记录、质量检测和功能性试验资料等内容。

五、填写要求：

1、建设工程中涉及的单项，应当在表格中对应单项栏目的“□”中打“√”；

2、单项验收由该单项的施工专业分包（或总承包）、监理单位加盖单位印章，同一单项涉及不同施工单位时，每家施工单位分别加盖单位印章；

3、综合评定由建设、设计、施工总承包、监理单位加盖单位印章；

4、按有关规定实行监理的建设工程，需填写“监理单位”栏目中的内容；

5、《工程竣工验收报告（消防）》由建设单位加盖骑缝章。
