Enterpriseemployeeinjury/illnessperformance
Weachieveda14%reductionin2013TotalRecordableIncidentRate(TRIR),surpassingour10%goalfortheyear.Although66fewerrecordableincidentsoccurredthaninthepreviousyear,580employeesstillsufferedarecordableinjury.InadditiontosuccessfullyloweringourTRIR,weachievedan18%reductioninourDaysAwayFromWorkCaseRate(DAFWCR).OurkeyHSEperformancemetricscontinuetooutperformtheInternationalAssociationofDrillingContractorsbenchmark,andareinthetopquartileacrosstheindustry(Fig.2).
Ourlosscontroltriangle(Fig.3)showsthatemployeessubmitted487,060observationsin2013,a20%increaseyear-on-year.Thisincreasecorrelatesdirectlywithourimprovedsafetyandenvironmentalperformance.
Injury/illnessdataanalysis
Figs.4and5illustratethetopfiveinjuredbodypartsandthesixmostcommontypesofinjuries
sustainedin2013.Handandbackinjuriescontinuetobethemostfrequentbodypartsaffected.Combined,theseinjuriesconstitute44%oftherecordableinjuriesfortheyear(Fig.4).Althoughsprainsandstrainscontinuetobethemostprevalenttypeofinjury,weexperienceda14%reductioncomparedto2012and31%comparedto2011(Fig.5).RecordableinjuriespredominantlyoccurredatthewellsiteorataBakerHughesfacility.Togethertheselocationsaccountedfor82%oftherecordableinjuries(Fig.6).
Transportationsafety
Ourtransportationsafetyperformanceimprovedlastyear.OurrecordableMotorVehicleAccidentfrequencyimproved4%,andouroverallaccidentfrequencyrate(allseverityclassifications)decreasedbymorethan9%.Whiletheseareveryencouragingresults,westillhavemanyopportunitiesforimprovement.Asinthepast,themajorityofourrecordableaccidentswereduetoacombinationoffactors,butexcessivespeedforroadandweatherconditions,improperfollowingdistance,poordriverawarenessofsurroundingsandinconsistentjourneymanagementpracticescontinuetobeourmostfrequentcontributors,reflectingtheneedforcontinuedtrainingandstrongoversightprograms.
Toaddresstheseissues,wecompletedathoroughdrivingtrainingprogramstrategyaddressingnotonlytheBakerHughesspecificdrivertrainingcomponents(coredefensivedriving),butalsodrivertrainersandassessors(TransportationTrainerandAssessorCertificationCourse).FurthersupportforthesetwoprogramswillcomewiththeintroductionoftheTransportationSafetyManagementLeadershipPrograminQ12014.Allthreeprogramswillalsobeintroducedglobally.And,wehaveinitiatedplanstopilotastate-of-theartdrivingsimulator.

During2013,weachievedastrongsafetyperformancewitha26percentdeclineininjuryrates.Thisisoneofthegreatestyearover-yearimprovementswehaveeverseen,anditspeakstothededication,allacrossourorganization,toensurethateveryonereturnshomesafe.
KeyAchievements
•AswecontinuetoworktowardourultimategoalofZERO,wereducedourtotalrecordableincidentrate(TRIR)toin2013.Thisrepresentsourbestperformance,anda70percentdecline,sincetheCompanybegankeepingconsolidatedrecordsin1999.Our2013lost-timeincidentrate(LTIR)ofrepresentsourbestperformance,andan86percentdecline,comparedto1999.
•Wereducedboththevolumeofspillsandourcarbonfootprintin2013.Additionally,weenhancedourcomprehensivechemicalindexandcontinuetoadvanceouremissionsreducing
dual-fueltechnology.
•Wecompletedafullupdateofourcorporatehealth,safetyandenvironmentalstandards.ThesestandardsdefinetheminimumexpectationsforthecontrolofanHSEactivityorsystem,andserveasabridgebetweenregulatoryrequirementsandHalliburton’soperations.

2.OVERVIEW
Anorganization’sHSEManagementSystemwillonlybeassuccessfulasthelevelofcommitmentdemonstratedbyitsleadershipandemployees.OneimportantmethodusedtogainandmaintainhighlevelsofcommitmentistheestablishmentofspecificHSErolesandresponsibilitiesthroughouttheorganization’sstructure.OperationalcontrolsaredesignedtomanageHSEaspects,risksandobligationsbutcontrolsareonlyasgoodastheindividualeffortstoimplementandmaintainthosecontrols.Establishingclearrolesandresponsibilityforthemanagementsystemandresultingcontrolswillhelptheorganizationimplementitsmanagementsystem,sustaincontroloveritsHSEaspectsanddrivecommitmentthroughouttheworkforce.ThisstandardwillhelporganizationsimplementandmaintainaneffectiveHSEmanagementsystembyestablishingclearlinesofresponsibilityandaccountability.

TheorganizationshallidentifyanddocumentHSErolesandresponsibilitiesnecessarytoestablishandmaintainaneffectiveHSEmanagementsystem.Rolesandresponsibilitiesmustbeestablishedforallemployeesincludingthefollowingorganizationlevels:

Lost-TimeCaseFrequencyRate(LTCFR).
