1.PURPOSES目的

Theaimofthisprocedureistomakemanagersandstaffawareofthedifferenttypesworkssituationsthatrequirefallarrestandothersafeguardsagainstfallingfromheight.

本程序旨在使经理和员工意识到不同类型的高处作业环境需要采取的防护设施及安全措施，避免人员从高空坠落而受到伤害。

2.SCOPE适用范围

ThisprocedureisapplicabletoHSEmanagementattheEPCorEPCMProjectSite.

本程序适用于EPC/EPCM项目现场HSE管理。

3.DEFINITIONS定义

1）100%Tie-Off100%系住

Theterm100%tie-offshallbeinterpretedasmeaningthatallemployees,atalltimes,shallbewearingapersonalfallarrestsystemandshallbesecurelyfastenedtoananchororfieldtie-offpointwhenworking,traveling,inspectingorperformingothertasksorconductingotherbusinessataheightgreaterthan2Mabovethegroundorworkinglevel.Theworkingheightshallbedeterminedbymeasurementfromtheemployee’sfeet.Completedandinspectedpermanentplatformsandgreen-taggedscaffoldareexemptfromthispolicy.

100%系住这个术语应解释为：

所有雇员在所有时间内在离地面或工作标高2米以上高度处进行工作、行走、检查或执行其它任务或完成其它工作时，都必须配戴人员高空坠落制动系统，并安全地固定在锚固件或现场系挂点上。

工作高度应从雇员的脚面测量来确定。

完整的经过检验的永久性平台和绿色标签的脚手架除外。

2）DecelerationDevice减速装置

Adecelerationdeviceshallbeinterpretedasmeaninganymechanism,suchasaropegrab,rip-stitchlanyard,specially-wovenlanyard,tearingordeforminglanyards,automaticself-retractinglifelines/lanyards,etc.,whichservestodissipateasubstantialamountofenergyduringafallarrest,orotherwiselimittheenergyimposedonanemployeeduringfallarrest.

减速装置应解释为任何机构，例如绳索夹钳、扯缝索、特殊的编结索、抗撕磨索，自动的自收缩救生索/系索等等，这些机构在坠落制动期间用来消散能量，或限制施加于雇员身上的能量。

3）EngineeredAnchorPoint设计的锚固点

Anengineeredanchorpointshallbeinterpretedasmeaningafixedlocation,designedandinspectedbyastructuralengineer（orequivalent）forthepurposeofconnectingasafetylanyardtopreventfalls.

设计的锚固点应解释为一个固定位置，该位置由结构工程师（或相当的人员）设计并检查，用于连接安全系索，防止坠落。

4）FieldTie-OffPoint现场系挂点

Afieldtie-offpointshallbeinterpretedasmeaninganyfixedpointcapableofsupporting10knofforce,usedfortheattachmentofasafetylanyard.Fieldtie-offpointscouldbepipeslargerthan300mminchesindiameterandstructuralbeams.Conduit,regardlessofsize,shallneverbeusedasatie-offpoint.

5）GuardrailSystem护栏系统

Guardrailsystemshallbeinterpretedasmeaningabarriererectedtopreventemployeesfromfallingtolowerlevels.

6）Harness全身式安全带

Aharnessshallbeinterpretedasmeaningawebbeddevicewhichissecuredaboutthebodyinamannerthatwilldistributethefallarrestforcesoveratleastthethighs,pelvis,waist,chestandshoulderswiththemeansforattachingittoothercomponentsofapersonalfallarrestsystem.Alsoknownasabodyharnessorsafetyharness.

7）PersonalFallArrestSystem人员高空坠落制动系统

PersonalfallArrestsystemmeansasystemusedtoarrestanemployeeinafallfromaworkinglevel.Itconsistsofananchorage,connectors,abodyharness,andmayincludealanyard,decelerationdevice,lifeline,orsuitablecombinationofthese.

人员高空坠落制动系统指的是用于防止雇员从高处坠落的系统。

它由锚固件、连接器、安全带组成，也可以包括绳索、减速装置、救生索，或这些器具的适当组合。

8）PositioningBeltorPositioningDevice定位带或定位装置

Apositioningbeltorpositioningdeviceshallbeinterpretedasmeaninganysystemriggedtoallowanemployeetobesupportedonanelevatedverticalsurface,suchasawall,andworkwithbothhandsfreewhileleaning.Nottobemisconstruedasasafetybelt.

定位带或定位装置应解释为指任何悬挂系统，使雇员支持在一个高空垂直表面，例如墙壁，并且手臂可以自由工作。

切勿误解为安全带。

9）SafetyBelt安全带

Asafetybeltisasinglewebbedorleatherbeltconstructedwithoneormore“D”ringsbuiltintothebackoronthesides.SafetybeltsarenottobeusedonTCCproperty.

安全带是一个单肩带或腰带，在背部或侧部装有一个或多个“D”形环。

这种安全带不用于TCC项目。

10）SafetyLanyard安全系索

Safetylanyardmeansaflexiblelineofrope,wirerope,orstrap,whichgenerallyhasaconnectorateachendforsecurelyattachingthebodybeltorbodyharnesstoadecelerationdevice,lifeline,oranchorage.

安全系索指一个柔性的绳索、钢索或带状绳索，通常在每端有一个连接器，将人体绳索或人体保险带牢固地连接到减速装置、救生索或锚固件上。

11）Self-RetractableLifeline/Lanyard自收缩式救生索/系索

ASelf-RetractableLifeline/Lanyardshallbeinterpretedasmeaningadecelerationdevice（seeabove）containingadrum-woundlinewhichcanbeslowlyextractedfrom,orretractedonto,thedrumunderslighttensionduringnormalemployeemovement,andwhich,afteronsetofafall,automaticallylocksthedrumandarreststhefall.

自收缩式救生索/系索应解释为指一个含有鼓状绕线的减速装置（见上），在雇员正常移动期间，它可在轻微的张力下从鼓状物缓慢地抽出，或缩回到鼓状物上；在坠落开始后，它可自动地锁住鼓状物并对坠落进行制动。

12）StaticLine固定绳

Astaticlineisanengineereddevice,typicallyacablethatisstrungfromtwopointstoprovideananchorpointforafallarrestsystem.Thestaticlineisusedtoallowsafetravelfromonepointtoanother.Staticlines,unlessotherwisenoted,aredesignedforsingleemployeeuseonly.

固定绳是一种象电缆一样的工程装置，它固定在两个点上，为坠落制动系统提供一个锚固点。

固定绳可以使人员安全地从一个点移动到另一个点。

除非另有说明，固定绳仅设计用于单一的雇员。

13）Toeboard踢脚板

Toeboardmeansalowprotectivebarrierthatwillpreventthefallofmaterialsandequipmenttolowerlevelsandprovideprotectionfromfallsforpersonnel.

踢脚板指的是一种低的保护屏障，防止材料和设备跌落到较低的平面，并对人员坠落提供保护。

14）UnprotectedSidesandEdges无保护的侧面和边缘

Unprotectedsidesandedgesmeananysideoredge（exceptatentrancestopointsofaccess）ofawalking/workingsurface,e.g.,floor,roof,ramp,orrunwaywherethereisnowallorguardrailsystem.

无保护的侧面和边缘指的是没有墙或护栏系统的步行/步行表面（例如楼面、屋顶、斜面或通道）的任何侧面或边缘（除了到出入点的入口之外）。

15）Walking/WorkingSurface步行/工作表面

Walking/workingsurfacemeansanysurface,whetherhorizontalorverticalonwhichanemployeewalksorworks,including,butnotlimitedto,floors,roofs,ramps,bridges,runways,formworkandconcretereinforcingsteelbutnotincludingladders,vehicles,ortrailers,onwhichemployeesmustbelocatedinordertoperformtheirjobduties.

步行/工作表面指的是雇员步行或进行工作的任何表面，无论水平的还是垂直的，包括但不限于楼面、屋顶、斜坡、桥梁、通道、模板和钢筋混凝土，但不包括梯子、车辆或拖车，雇员必须在上面执行工作任务。

4.PROCEDURE规定

4.1RISKASSESSMENT风险评估

CMisrequiredtoevaluaterisksfromworkingatheightsandadoptworkingpracticesthatwillreducethelikelihoodofthepotentialconsequences.Ariskassessmentistobecarriedoutandsomeconsiderationsarelistedbelow.Workcarriedoutatheightspresentpotentialconsequencessuchas:

施工经理应对高处作业的风险进行评估，采取适当的工作程序和条例，降低潜在危险发生的可能性。

作业之前应进行风险评估，并考虑以下因素。

高处作业存在潜在后果，例如：

a）Fallsbypersonnelfromworkingplatforms人员从工作平台坠落下来。

b）Droppedobjectsfromworkingplatforms物体从工作平台上掉落下来。

c）Structuralcollapseofworkingplatforms.工作平台倒塌。

Themainconsiderationstobeassessedare:

评估时应考虑以下主要因素：

a）Selectionofthemostappropriatetypeofworkingplatform选择最合适的工作平台。

b）Platformsdesignedandconstructedfortheworkingloadsinvolved（includingclimaticconditionssuchaswindloading,wherenecessary）根据有关的工作负荷（若需要应考虑气候条件，如风载）设计和建造工作平台。

c）Protectionforpersonnelgainingaccesstoplatformsatheight保护人员安全到达高空平台。

d）Protectionforpersonnelinthecourseofcarryingouttasksandoperationsatheight保护在高空执行任务和进行作业的人员。

e）Protectionagainstaccidentaldislodgmentoftools,materialsandequipmentfromworkingplatforms防止工具、材料和设备从工作平台上意外坠落。

f）Correctuseofstabilizingmechanismsforattachmentstopermanentstructures正确使用稳定机械装置，将附件连接到永久结构上。

g）Protectionofpersonnelequipmentfromdroppedobjectsfromworkingplatforms保护人员、设备不受工作平台上坠落物的伤害。

h）Platformsbuiltanddismantledinthesequencedeterminedbythedesign根据设计顺序安装和拆卸工作平台。

i）Platformsloadedwithinthedesignlimitations工作平台的承重限制在设计范围内。

j）Platformsprotectedagainstphysicalimpactbyvehicles,constructionplantorequipment.保护工作平台免受车辆、施工机器或设备的碰撞而损坏。

4.2FALL-PROTECTIONPLANS坠落保护计划

Thecontractorshalladopteffectivemeasurestopreventpersonnelformfalling.Requirementsaregivenwhereapersonisliabletofalltwometersormorerequiresthatsuitableandeffectivemeasuresbetakentopreventanypersonfallingadistancelikelytocausepersonalinjury.

承包商必须采取有效措施，防止人员坠落。

当人员可能从2米以上高空坠落时，必须采取合适而有效的措施，防止坠落对人员造成伤害。

Otherconsiderationsinclude:

其他需要考虑的事项，包括：

–theindicationofareaswhereafallcouldoccur注明可能发生坠落的区域

–guardrails护栏

–fixedladders固定梯子

–roofwork屋顶作业

–fragilesurfaces易碎表面

–windowcleaning窗户清洁

–fall-arrestsystems,includingsafetylines,harnessesandsecureanchoragepoints

Usethefollowingguidelineswhenplanningworkatheights:

在编制高空作业计划时要遵循以下方针：

a）Requirecontractorstopreparewrittenfallprotectionplans,especiallywhentheydoroofingwork,constructcoolingtowers,erectsteelstructuresandfabricateanderecttanksandboilers.

要求承包商准备书面的坠落保护计划，特别是当他们要进行屋顶作业、冷却塔施工、安装钢结构和预制、安装储罐和锅炉时。

b）Specifyandinsistthatfabricatorssupplypermanentstairsandguardrailsbeforesteeliserected.Inthisway,thestairsandsteelcanbeerectedsimultaneously,improvingnotonlysafetybutalsoefficiency.Thesteelworkerswillhavesafeaccessandegressandthenecessityofclimbingtemporaryladderswillbereducedoreliminated.

在钢结构安装前必须按规定给安装人员提供永久性楼梯并且要有扶手。

这样，楼梯和钢结构可以同时安装，既安全又高效。

钢结构安装人员有了安全的通道和出口就必然会减少或不使用临时性梯子。

c）Makesurethattemporaryperimeterprotectioniseffective.

要确保临时性周边防护的有效性。

d）Attachlifelinestosteelcolumnswhilethesteelisonthegroundtoprovidetie-offforsteelworkers,unlessothermethodsoffallprotectionareprovided.

当钢结构在地面时要在钢柱上面安装救生索为钢结构作业人员提供安全带系挂点，除非能提供其他坠落保护措施。

e）Provideworkplatformsforelectricianswhoinstallcabletraysandpullwiresintoelevatedcabletrays.

要给安装电缆和放电缆的电工提供工作平台。

f）Usecompletescaffoldsandaerialliftstoprovidesafeworkplatforms.

要使用完善的脚手架为高处吊装作业提供安全的工作
