建筑施工企业安全生产责任承诺书
遂宁市建设局：
根据《中华人民共和国安全生产法》、《中华人民共和国建筑法》、《建筑工程安全生产管理条例》和《建筑工程安全生产监督管理工作导则》的相关规定，我公司将对明泰-米兰星城工程中的安全生产实施全过程全方位的管理，加大检查力度，增加安全投入，消除各类安全隐患，认真履行以下安全生产管理责任。
1、严格按遂宁市建设局“关于转发四川省建设厅《关于转发财政部、国家安监总局的通知》”的通知要求，将按该工程造价的2%计取安全生产费，并保证此项经费专户核算、按施工进度使用，同时还自愿接受上级主管部门和监理单位对此项费用的使用进行监督。
2、建立安全生产责任制度和安全生产教育培训制度，制定安全生产规章制度、安全生产检查制度和操作规程，保证对该工程定期进行专项安全检查，并做好安全检查记录。
现委托项目经理，对该工程项目的安全施工负责，落实安全生产责任制度、安全生产规章制度和操作规程，确保安全生产费用的有效使用，并根据工程的特点组织制定安全施工措施。
消除安全事故隐患，及时、如实报告生产安全事故，并保证每周在施工现场履职时间按招投标合同中的要求执行。
3、委派同志担任该工程的专职安全生产管理人员，负责对安全生产进行现场监督检查。
发现安全事故隐患，应当及时向项目经理和安全生产管理机构报告；对违章指挥、违章操作的，应当立即制止，且保证其每周常驻现场不少于6天。
电工、电焊工、塔吊司机、垂直运输机械作业人员、安装拆卸工、起重信号工、登高架设作业人员等特种作业人员持有特种作业操作资格证书，方允许其上岗作业。
4、按照建设部“建质[2004]213号”规定，对施工过程中含有危险性较（重）大的分部（分项）工程，必须严格执行专项施工方案的编制、审核、审批程序，在施工时还应做好安全技术交底和检查验收工作。
对危险性重大工程（重大危险源工程）的专项施工方案，还应组织不少于5人（本专业工程的高级工程师）的专家组对已编制的方案进行论证审查，且要有专家组作出书面论证的审查报告。
对各专项施工方案应具备的相关要素详见《相关分项工程施工方案提示单》（附
七）。
5、该工程施工前，我公司负责项目管理的技术人员对有关安全施工的技术要求向施工作业班组、作业人员作出详细说明，并由双方签字确认。
对该工程施工现场的出入口处、施工起重机械、临时用电设施、脚手架、出入通道口、楼梯口、电梯井口、孔洞口、基坑边沿等危险部位，设置安全防护和明显的安全警示标志，安全警示标志的设置必须符合有关标准。
6、施工现场的办公、生活区与作业区做到分开设置，并保持安全距离；办公区、生活区的选址必须符合相关安全性规定要求。
职工的伙食、饮水和休息场所应当符合卫生标准。
坚决杜绝在尚未竣工的建筑物内设置员工集体宿舍。
施工现场临时搭建的建筑物和施工场地、道路及施工围墙的设置按《遂宁市施工现场管理规定》执行。
7、对因施工可能造成损害的毗邻建筑物、构筑物和地下管线等应当采取专项防护措施，并遵守环境保护有关的法律法规。
在施工现场采取措施，防止或者减少粉尘、废气、废水、固体废物、噪声和施工照明对人和环境造成的危害和污染。
在施工现场建立消防安全责任制度，确定消防安全责任人，制定用火、用电、使用易燃易爆材料等各项消防安全管理制度和操作规程，设置消防通道、储备消防水源、配备消防设施和器材，并在施工现场作业区设置明显标志。
8、认真做好“三级”安全教育工作，作业人员进入新的岗位或者新的施工项目开工前，必须接受安全生产教育培训。
未经教育培训或者教育培训考核不合格的人员不得上岗作业；同时要向作业人员提供必要的安全生产防护用具和用品，并告知危险岗位的操作规程和违章操作的危害。
一旦在施工中发生危及人身安全的紧急情况时，立即停止作业，采取必要的应急措施或撤离危险区域。
教育施工作业人员遵守安全施工的强制性标准、规章制度和操作规程，正确使用安全防护用具、机械设备等。
在采用新技术、新工艺、新设备、新材料时，也要对作业人员进行相应的安全生产教育培训。
9、采购、租赁的安全防护用具、机械设备、施工机具及配件，应具有生产（制造）许可证、产品合格证，并在进入施工现场前进行查验。
施工现场的安全防护用具、机械设备、施工机具及配件等由专人管理，定期进行检查、维修和保养，确保其性能指标处于良好的工作状态。

10、所使用的各种施工起重机械和整体提升（自升式）脚手架、模板等危险性较大工程（设备）在安装、搭设、使用前，应组织有关单位对其安全性能进行验收，或委托具有相应资质的检验检测机构进行验收；使用承租的机械设备和施工机具及配件的，由施工总承包单位、分包单位、出租单位和安装单位共同进行安全性能验收。
验收合格后方可使用，并按相关规定做好登记备案工作。
11、认真抓好安全监督部门对该项工程存在事故隐患下达相应执法文书的整改、复查及回复工作，坚决做到令行禁止，不违法违规施工。
12、扎实做好该项目部所有从业人员的维权工作，保证所有从业人员的工资按合同约定及时、足额发放。
我代表公司承诺：
严格执行上述内容及标准抓好该项工程的安全管理，随时接受江宁区建筑安全生产监督站的监督。
如果违反相关法律法规的条款造成不良社会影响或者严重后果的，本公司自愿接受建筑行政主管部门参照《建筑工程安全生产管理条例》和《生产安全事故报告和调查处理条例》实施的各项行政处罚，且承担相应的法律责任。
