Thepatient,R.S.,wasa54-year-oldAfricanAmericanmalewithhistoryofsevereCOPD;hewasadmittedwithanacuteexacerbationofCOPDandhypoxemicrespiratoryfailure.Onadmission,hewasinacuterespiratorydistress,hisrespiratoryratewas37,pulse136,bloodpressure100/62,andSpO2viapulseoximetrywas82%.Onexamination,R.S.hadanincreasedwork-of-breathingandwasusingaccessorymuscles.Hehaddecreasedairentryonlungexam.HisinitialchestX-rayshowedhyperinflatedlungs.ThepatientwasintubatedonadmissionandtransferredtotheICU.Duringhisstayhewastreatedwithhigh-dosesteroidsforhisCOPDexacerbationandhadtobesedatedandparalyzedinordertomaintainanoxygensaturationabove90%onventilatorysupport.Asthepatientimproved,thesystemiccorticosteroiddosewastaperedandthesedationwasweaned.Earlyinhishospitalcourse,thepatientwasnotacandidateforphysicaltherapyduetohismedicalinstability.Atthepresenttime,thepatient’sbodyisrotatedevery2hbythenurse.Multipodusbootsandhandsplintswereplacedonthepatienttopreventjointcontractures.
