Thatnightbeganmyformalrelationshipwithdeath--mydeath--anditalsobeganmylongrunasapatient.It'

maphysiciannow,ahospiceandpalliativemedicinedoc,soI'
veseencarefrombothsides.Andbelieveme:
almosteveryonewhogoesintohealthcarereallymeanswell--Imean,truly.Butwewhoworkinitarealsounwittingagentsforasystemthattoooftendoesnotserve.
Why?
Well,there'
sactuallyaprettyeasyanswertothatquestion,anditexplainsalot:
becausehealthcarewasdesignedwithdiseases,notpeople,atitscenter.Whichistosay,ofcourse,itwasbadlydesigned.Andnowherearetheeffectsofbaddesignmoreheartbreakingortheopportunityforgooddesignmorecompellingthanattheendoflife,wherethingsaresodistilledandconcentrated.Therearenodo-overs.
Mypurposetodayistoreachoutacrossdisciplinesandinvitedesignthinkingintothisbigconversation.Thatis,tobringintentionandcreativitytotheexperienceofdying.Wehaveamonumentalopportunityinfrontofus,beforeoneofthefewuniversalissuesasindividualsaswellasacivilsociety:
torethinkandredesignhowitiswedie.
Solet'
sbeginattheend.Formostpeople,thescariestthingaboutdeathisn'
tbeingdead,it'
sdying,suffering.It'
sakeydistinction.Togetunderneaththis,itcanbeveryhelpfultoteaseoutsufferingwhichisnecessaryasitis,fromsufferingwecanchange.Theformerisanatural,essentialpartoflife,partofthedeal,andtothiswearecalledtomakespace,adjust,grow.Itcanbereallygoodtorealizeforceslargerthanourselves.Theybringproportionality,likeacosmicright-sizing.Aftermylimbsweregone,thatloss,forexample,becamefact,fixed--necessarilypartofmylife,andIlearnedthatIcouldnomorerejectthisfactthanrejectmyself.Ittookmeawhile,butIlearnediteventually.Now,anothergreatthingaboutnecessarysufferingisthatitistheverythingthatunitescaregiverandcarereceiver--humanbeings.This,wearefinallyrealizing,iswherehealinghappens.Yes,compassion--literally,aswelearnedyesterday--sufferingtogether.
Now,onthesystemsside,ontheotherhand,somuchofthesufferingisunnecessary,invented.Itservesnogoodpurpose.Butthegoodnewsis,sincethisbrandofsufferingismadeup,well,wecanchangeit.Howwedieisindeedsomethingwecanaffect.Makingthesystemsensitivetothisfundamentaldistinctionbetweennecessaryandunnecessarysufferinggivesusourfirstofthreedesigncuesfortheday.Afterall,ourroleascaregivers,aspeoplewhocare,istorelievesuffering--notaddtothepile.
Truetothetenetsofpalliativecare,Ifunctionassomethingofareflectiveadvocate,asmuchasprescribingphysician.Quickaside:
palliativecare--averyimportantfieldbutpoorlyunderstood--whileitincludes,itisnotlimitedtoendoflifecare.Itisnotlimitedtohospice.It'
ssimplyaboutcomfortandlivingwellatanystage.Sopleaseknowthatyoudon'
thavetobedyinganytimesoontobenefitfrompalliativecare.
Now,letmeintroduceyoutoFrank.Sortofmakesthispoint.I'
vebeenseeingFranknowforyears.He'
slivingwithadvancingprostatecancerontopoflong-standingHIV.Weworkonhisbonepainandhisfatigue,butmostofthetimewespendthinkingoutloudtogetherabouthislife--really,aboutourlives.Inthisway,Frankgrieves.Inthisway,hekeepsupwithhislossesastheyrollin,sothathe'
