8、7)结合临床用药，开展药物评价和药物利用研究。 4、医务人员如发现可能与用药有关的严重不良反应，在做好观察与记录的同时，应及时报告本机构药学部门和医疗管理部门，并按规定上报药品监督管理部门和卫生行政部门。 5、药学专业技术人员发现处方或医嘱所列药品违反治疗原则，应拒绝调配;发现滥用药物或药物滥用者应及时报告本机构药学部门和医疗管理部门，并按规定上报卫生行政部门或其他有关部门。 6、医疗机构开展新药临床研究必须严格执行国家卫生行政部门和国家药品监督管理部门的有关规定。未经批准，任何医疗机构和个人不得擅自进行新药临床研究。违反规定者，将依法严肃处理，所获数据不得作为新药审批和申报科技成果依据。 四

9、、临床药学室工作制度 1、本室根据医疗、教学、科研的需要，配备相应的临床药学技术人员、设备、图书等，积极开展工作。 2、临床药学工作应由经验丰富，业务水平高的药学技术人员担任。 3、临床药学工作人员要有高度的责任心和严谨的科学态度，努力学习，不断提高业务知识水平和专业技术水平。 4、本室应结合实际，积极开展处方病历分析、药品不良反应监测以及血药浓度监测等工作。 5、临床药学技术人员参加查房、会诊、疑难病历讨论，对临床用药提出科学合理的建议。 6、为配合临床治疗需要，积极开展治疗药物监测，为临床个体化给药提供方案。 7、积极开展用药咨询，建立相应的记录。 provided these measu

15、疗费用的影响; (3)受临床医师采纳、支持及好评的程度。 2、深入临床实践选定定点科室，参加每周主任查房、病历分析讨论、教学等专业活动，每年参加临床实践工作时间不得少于2640周，平均每周在临床参与临床用药相关工作的实践时间不得少于5080%工作日，每天书写查房记录，按月对查房记录进行整理汇总;阅读病历、对典型病例书写“药师临床查房、会诊记录”，查阅服药与治疗用药医嘱记录，完成药历不少于12份/年人;进行专科用药调查分析，收集与反馈有关药物信息，学习专科疾病的特点与用药规律，并积极配合临床，做好临床和药剂科之间的协调工作。 3、服务非定点科室，非定点科室需要临床药师参与工作时做到及时到位外，应

16、完成每周一次的非定点科室寻访，进行用药调查，查阅药物治疗与医嘱记录，阅读重点病历，了解危重病人的用药情况。 4、节假日前临床药师应有计划重点深入科室，对监护病人的治疗用药进行寻查，必要时向医生提出调整治疗方案的建议，协调做好急救药品的准备。 5、参与临床抢救会诊。临床药师接到医务科或专科提出的会诊通知后，全院会诊按时(通知)到位，急救会诊10分钟内到位，一般会诊当日完成。 6、制订个体化给药方案。通过参加查房、会诊，依据病情、病理生理学、药理药效学、病原学以及生化检验资料数据等，参与典型病例个体化给药方案的制订，并协助方案的执行、修改与评价。 7、重点病例的药学监护。对药师参与会诊抢救或提供了

17、个体给药方案的重点病例，必须每日或隔日深入病房，直接面对病人参与监护，观察并记录病情变化，药物疗效，不良反应情况等，并认真阅病历及治疗记录，进行疗效评价，必要时提出修正用药方案建议，直至病情稳定、个体化治疗方案结束并转入专科常规治疗后方可结束重点药学监护。 8、药品不良反应(ADR)监测。临床药师每周深入临床了解与收集有关ADR情况，协助临床预防严重ADR病例的发生，参与ADR救治处置。负责ADR报provided these measures 1, authentication implement the industrial science and technology policy, a

23、药事管理暂行规定，根据四川省卫生厅医院等级评审实施细则及我院质量管理年活动的有关规定及要求，建立适合我院实际情况的医、药、护互相协作，提高医疗质量的工作方式，规范临床药师工作模式，特制定本制度。 1、资格:参与会诊的药师由副高职称或临床药师担任。 2、临床药师接到会诊通知后，按时(通知)到位，急救会诊10分钟内到位。药师提供抢救治疗用药方案的建议，经会诊组讨论通过后方可执行。 3、仔细阅读病历，科学严谨地进行用药分析。 4、对会诊后的病人，进行跟踪查房，对其给药方案的有效性和安全性进行观察，协助医生制定合理给药方案。 六、临床药师查房制度 为贯彻落实卫生部医疗机构药事管理暂行规定，根据四川省卫

24、生厅医院等级评审实施细则及我院质量管理年活动的有关规定及要求，建立适合我院实际情况的医、药、护互相协作，提高医疗质量的工作方式，规范临床药师工作模式，特制定本制度。 1、临床药师深入临床参与查房，规范执业。 2、坚持每天到病房巡视病人，了解病人病情和用药情况。 3、参加医师查房，在查房讨论中提出科学合理的用药建议。 4、对病人用药进行指导，询问和观察用药后的情况，了解药物相互作用，药物和食物相互作用，保证安全用药，记入药师查房记录。 5、对特殊病人的用药进行指导。 6、参与危重病人的抢救。 7、向医生、护士以及病人提供药物咨询。 provided these measures 1, authe

30、药学专家担任。 2、药物咨询人员负责解答患者提出的有关药品名称、剂型、剂量、使用方法、适应症、禁忌症、慎用症、特殊人群(孕妇、哺乳妇女、儿童、老年患者)用药注意事项、药品不良反应、药物间的相互作用、药品的贮存及有效期等与药学有关的问题。 3、负责解答其他相关的药学保健问题，必要时应向临床医师咨询或查找资料后再做答复。 4、记录咨询内容，注明解决的问题，若属药物不良反应要及时登记、报告。 5、定期汇总药物咨询内容，归纳典型案例向其他药师反馈交流，并适时在全科报告。 6、门诊药房设立药物咨询窗口，其他未设咨询窗口的部门按“药剂科首问负责制”要求开展药物咨询工作。 7、若门诊咨询药师不在时，其他药师

31、实行“首问负责制”，行使咨询药师职责。 8、专科临床药师为所在病区的住院患者用药咨询指导，向医生提供咨询，向护士提供服务，为社会提供药学服务， 做好记录，临床随访。 九、临床药师岗位职责 1.在分管临床药学工作的主任领导下工作。 2.做好用药咨询、抽查处方合格率的工作，并结合临床做好合理用药分析。 3.应定期深入临床一线，参加查房及疑难病历讨论、危重病人抢救工作，负责监督临床的合理用药。 4.组织检查药品不良反应登记报告制度执行情况，及时汇总药物不良反应，上报不良反应监测中心。 5.积极开展处方，病历分析、血药浓度监测工作，结合临床制定个体化给药方provided these measures
