Purpose目的1.1 The procedure is designed to protect employees and contractors from hazards associated with the unexpected startup or release of stored energy in machines or equipment. 本程序旨在保护员工和承包商免受机器或设备意外启动或储能释放带来的伤害。1.

4、2 Lock out Tag out (LOTO) is the means by which operations, maintenance, or other personnel may be assured that a potentially dangerous system is properly shut down, isolated, and verified prior to starting service or maintenance activities. 上锁挂牌指操作、维修或其他人员能够确保潜在威胁系统能够在开始服务或维修活动前被适当的关闭、隔离并确认的方式。2. 6、s procedure applies to people who may be exposed to any type of energy during service or maintenace activities, including PNAN assocaites and third party personnel. 当PNAN公司员工与第三方人员在对机器/设备/系统实施干预时暴露于能量源的情况下时,应遵守本程序。3.2 The procedure applies to the control of energy, including electrical, mechanical,

7、hydraulic, pneumatic, chemical, thermal, potential, kinetic, and any other form of energy, during servicing or maintenance of machines and equipment. 本程序适用于在对机器和设备服务或维修时对能源的控制,包括电能、机械能、液压能、气压能、化学能、热能、势能、动能和其他形式的能源。4. Definition 定义4.1 Energy能量:All types of energy that may cause injuries or losses. Ty

8、pical energy souces include: 所有可能造成损伤或损失的能量。典型的能源类型包括:上图从左至右依次为电能、气压能、机械能及液压能。4.2 Hazardous areas 危险区域:Hazardous areas are the machines/equipments/systems in which accidents could occur due to unexpected or inadvertene machine movements. 危险区域指的是因为设备的意外动作可能导致事故发生的区域。4.3 Energy Isolating Device (EID)

9、能量隔离装置: Mechanical devices that physically prevent transmission or release of energy. Examples of EID: circuit breakers, master switches, valves, push buttons, plugs, etc. Emergency push buttons are not considered as EID. 防止能量传递或泄露的装置,例如断路器、主控开关、阀门、按钮、插座等。急停按钮不作为能量隔离装置。4.4 Energy Source Analysis For

10、m (ESA Form) 能源分析表: This form is used to identify all energy sources in the machines/equipments/systems, providing guidelines for switching off and locking, as well as to specify the location of each EID.For the format of ESA Form, refer to F-HS-SF-002 Energy Source Analysis Form format. 能源分析表用于识别机器

11、设备上的所有能量源,提供关机、上锁指导,标明各个能量隔离装置的位置。参见F-HS-SF-002能源分析表模板。4.5 Lock device 锁具:Padlock, multi-lock hasps, and other lock device applied on Energy Isolating Device (EID), ensuring that EID being controlled cannot be operated until the lock device is removed. 指挂锁、锁扣及其他锁具,应用于能源隔离装置(EID)上,确保其受控,在锁具移除前无法运行。 p

13、y found in the machines/equipments/systems even after it is disconnected from sources of enery supply. 即使在关闭或隔离机器设备之后,其中仍然储存的过剩能量。4.8 Zero Energy State零能量状态: All energies (feed and residual) have been duly isolated or exhausted. 所有能量(外部供能、内部储能)被妥善隔离或释放。4.9 Intervention干预:In this procedure, intervent

14、ion is decribed as inspection, set-up, cleaning, greasing, adjusting,repairing, installing, disassemblying and replaceing on machines or equipments. 本程序中,服务或维修是指在机器设备上进行的检查、调试、清洁、加油、调整、维修、更换、安装或拆除。4.10 Authorized Person授权人员: An individual qualified to perform lock out/tag out by virtue of training a

15、nd demonstrated competency. 经培训并具有执行上锁挂牌程序能力的人员。4.11 LOTO Leader 上锁挂牌领导人员:An Authorized Person who leads the intervention. When there is more than one participant in the intervention, a Intervention leader must be identified and communicated to the entire intervention team. Paticipate and monitoring

16、 all the work all the time. 领导上锁挂牌的授权人员。当有多人共同执行干预任务时,必须选出一人作为上锁挂牌的领导人员,并通知上锁挂牌工作小组全体人员。全程参与并监视整个工作。4.12 Affected Person受影响人员:Anyone who either works or may work in an hazardous area where lock out/tag out is performed.可能在执行上锁挂牌程序区域的人员。4.13 Job Risk Analysis风险分析:Identifying all work steps, estimatin

17、g risk and its extent in each step, proposing controlling measures to eliminate or minimize risk. 识别所有工作步骤,评估每一步骤的风险及其程度,提出控制措施的建议以消除或降低风险。5. Lock out Tag out Requirement for Machine Manufacture对机器制造商的上锁挂牌要求5.1 Machine should design reliable to ensure no more intervention. It also should consider ma

18、king maintenance technician exposure to hazardous energy as less as possible when do routine maintenance. 制造商设计的机器应可靠,以确保无需多干预。机器的设计应尽可能使得从事例行和重复保养时维护人员不会暴露在危险能量中。5.2 For the intervention with energy, the manufacture should conduct risk assessment and provide other control method to define the safes

19、t way to enter machine and ensure people safe. The manufacture should provide user manual and should state every step clearly. 对于需要进行带能的干预,制造商应进行风险评估,提供其他控制方法,以确定人员进入机器的最安全方法,确保人员安全。应在制造商提供的使用手册中详细说明每一步程序。5.3 Energy Isolating Device (EID) 能量隔离装置5.3.1 EID should be considered according to the energy

EID should be part of machine and can control or release hazardous energy. 设备在制造时应根据其所含能量考虑能量隔离装置。能量隔离装置应该是设备的组成部分,并能够有效控制或消散危险能量。5.3.2 EID should be installed easy access area and outside danger zone and easy perform lock. 能量隔离装置应当安装在容易接近的地方,位于危险区之外并便于上锁。5.3.3 New purchasing

21、equipment should require identify IED, the identification should include service machine, energy type and magnitude. 采购新设备可要求标识能量隔离装置,应包括所服务的机器,能量的种类和大小。6. Lock out Tag out Preparation上锁挂牌准备6.1 Energy Source Analysis (ESA)能量源分析表 6.1.1 Energy Source Analysis (ESA), which lists all energy sources, loc

22、ate the energy isolating device (EID) to which the lock out devices will be attached, which kind of lock out device should be used, how to release residue energy, how to check the energy is released completely. 能量分析表列出所有能量源,指出需要上锁的能量隔离装置的位置,使用何种锁具,怎样释放能量以及怎样检查释放完全。6.1.2 Every machine or equipment mu

23、st have ESA.任何一台机器和设备必须要有能量源分析表。6.1.3 Maintenance team(maintenance team refers to Machining Maintenance team, Forging Maintenance, and team with maintenance task in Plant Engineering department, the same below) is responsible for conducting ESA . HSE review ESA and Maintenance Manager/Plant Engineer

24、ing Manger validate ESA document. 维修团队(是指机加工维修团队,锻造维修团队,工厂工程部负责维修的团队,下同)负责能量源分析表的编写。 HSE负责回顾能源分析表,维修经理和工厂工程部经理应当批准。6.1.4 Maintenance team should review and update ESA when any changes are implemented to machines or equipment. 当设备或机器有变化时维修团队应该回顾并更新能量源分析文件。6.2 Energy Isolating Devices (EID) Labeling能源

25、隔离装置标签6.2.1 All energy isolating devices (EIDs) that might be used during the LOTO implementation must be permanently and legibly labeled. 在执行上锁挂牌时所有能量隔离装置必须有固定的和易读的标签。6.2.2 Maintenance team is responsible for EID labels.维修团队负责能量隔离标签。Label format is as below:标签模板如下:6.2.2.1 Color :organe. 颜色:桔色。6.2.2

26、.2 The first number is based on the energy type. 27、umber will be used for identification.如果同类能源的EID多于一个,则需要在标签中设置第二个数字以示区分。Example:One electrical EID is marked as “1-1”, a second one is marked as “1-2”, a third one is marked as “1-3”.例如,一个电能隔离装置标作“1-1”,第2个电能隔离装置标作“1-2”,第3个电能隔离装置标作“1-3”。6.3 LOTO device 上锁挂牌装置6.3.1 LOTO device include lock device and

28、tags.上锁挂牌装置包括锁具和吊牌。6.3.2 Lock device includ pad locks, multi-lock, ball valve lock, butterfly valve lock, group lock, wire rope, circuit breaker lock , plug lock, cylinder valve lock, cloth bag lock and so on. 锁具包括通挂锁,六联锁,球阀锁,蝶阀锁,群锁,钢缆锁,断路器锁,插头锁,气瓶锁,布袋锁等6.3.3 HSE is resposible for slecet proper LOTO

29、 device and purchasing in warehouse. HSE负责选择合适的上锁挂牌装置并且采购到仓库。6.3.4 Regarding personel tag, department should collect information and provide to HSE. 关于个人吊牌,部门应收集信息并且提供给HSE。6.3.5 Using department get LOTO device from warehouse and maintain properly. 使用部门从库房领取上锁挂牌装置并且适当维护。6.3.6 LOTO device should stor

30、age in proper place on site and keep clean and orgnized. 现场上锁挂牌装置应储存在适当位置并且保证干净和整洁。6.3.7 Distrubute personal lock and tag to autherized person. People should be respobsible for personnal lock. If personal lock is missing or damaged , should pay for it. 对授权人员发放个人挂锁和吊牌。个人对自己的挂锁负责,如果遗失或损坏应赔偿。7. Lock ou

31、t Tag out Implementation 上锁挂牌的实施It is mandatory to apply the seven steps to conduct lock out/ tag out:上锁挂牌必须遵守以下7个步骤:7.1 Identify all energy sources识别所有能量源;7.1.1 According to job content and ESA to identify the energy may cause injury. 根据工作内容和能源分析表识别出可能会造成伤害的能源。7.1.2 When arranging intervention on m

32、achines/equipments/systems without ESA, a job risk analysis must be conducted and approved by maintenance supervisor and HSE. 如果待干预的设备未进行能量源分析,则必须进行工作风险分析并经维修主管人员和HSE批准。7.2 Turn on all EIDs(Energy Isolating Devices)关闭所有能量隔离装置(隔离能源);7.2.1 According to ESA to find the EID location and switch off. 根据能源分析表找到能量隔离装置并关闭
