9、afety, protecting fire control facilities, fire prevention, reports of fire附件1煤矿安全质量标准化考核评级办法（试行）第一条 为深入开展全国煤矿安全质量标准化工作，根据安全生产法、国务院关于进一步加强企业安全生产工作的通知（国发201023号）和国务院安委会关于深入开展企业安全生产标准化建设的指导意见（安委20114号）等法律法规、规定，制定本办法。第二条 本办法适用于全国所有合法的生产煤矿，新建、技改（包括重组整合）煤矿参照执行。第三条 考核评级标准执行煤矿安全质量标准化基本要求及评分方法（试行）。第四条 申报安全质

10、量标准化煤矿的基本条件：1、 证照齐全有效。2、 实现安全生产目标：考核年度内达到安全生产目标要求。3、 隐患排查治理：按照安全生产事故隐患排查治理暂行规定（国家安全监管总局令第16号）建立安全生产隐患排查治理体系。4、 采掘关系正常：开拓煤量、准备煤量、回采煤量、抽采煤量符合有关规定，回采率达到要求。5、 自查考核奖惩：煤矿企业制定并执行安全质量标准化考核评比及奖惩制度。6、 按要求建立煤矿瓦斯综合治理工作体系。第五条 安全质量标准化煤矿分为三个等级。一级：煤矿安全质量标准化考核评分90分及以上，且年度内无死亡事故。井工煤矿通风、地测防治水、采煤、掘进、机电、运输的单项考核评分均不低于90分

11、，其他专业均不低于80分；露天煤矿穿孔、爆破、采装、运输、排土、机电、边坡的考核评分均不低于90分，其他专业均不低于80分。二级：煤矿安全质量标准化考核评分80分及以上，且井工煤矿百万吨死亡率低于全国及所在省（直辖市、自治区）上年度平均水平，露天煤矿年度内无死亡事故。井工煤矿通风、地测防治水、采煤、掘进、机电、运输的单项考核评分均不低于80分，其他专业均不低于70分；露天煤矿穿孔、煤破、采装、运输、排土、机电、边坡的考核评分均不低于80分，其他专业均不低于70分。三级：煤矿安全质量标准化考核评分70分以上，且百万吨死亡率低于所在省（直辖市、自治区）上年度平均水平。井工煤矿通风、地测防治水、采煤

12、、掘进、机电、运输的单项考核评分均不低于70分，其他专业均不低于60分；露天煤矿穿孔、爆破、采装、运输、排土、机电、边坡的考核评分均不低于70分，其他专业不低于60分。第六条 考核检查过程中发现申报煤矿有煤矿安全质量标准化基本要求及评分方法（试行）总则所列的重大安全生产隐患和行为时，应当立即责令其停产整改，待隐患排除后，重新申报。第七条 鼓励煤矿企业采用煤矿安全风险预控管理体系规范（AQ/T10932011）开展安全质量标准化创建工作，其指标与煤矿安全质量标准化基本要求及评分方法（试行）对标确认，报国家煤矿安全监察局备案后可引用，但考核评级工作须按照本办法执行。第八条 煤矿安全质量标准化等级按年度分级考核。一级安全质量标准化煤矿的考核工作由国家煤矿安全监察局负责，具体的评审工作委托中国煤炭工业协会承担；二级、三级的考核工作由省级主管部门负责。第九条 煤矿安全质量标准化考评，按照企业申报、现场考核、等级认定、公示发布、颁发证书的程度进行。第十条 安全质量标准化煤矿的检查考核。1.安全质量标准化煤矿考核采取动态检查。全国煤炭行业每年进行一次；省（直辖市、自治区）每半年抽查1次；矿每月进行1次全面自查。2.对被取消和未
